从脏腑学说探讨儿童发声性抽动发病机制

摘　　要：发声性抽动主要病变部位在咽喉。咽喉连接口鼻与肺胃,同时是多条经脉循行所过之处。因而,发声性抽动虽与五脏皆相关,但主要在肺胃。肺胃气机升降失调,肺气不宣,胃气不降,则咽喉不利,致咽痒、干咳;肺脾气虚,无以推动血行,咽喉失于濡养滋润,易发吭吭声;肺脾失运,水液代谢失调,痰饮邪毒壅滞咽喉,致喉间痰鸣怪叫。风为百病之长,善行数变,易引动肝风而致发声异常。临证应四诊合参,辨证论治。The main site of vocal tics is in the throat. The throat connects the mouth,nose with the lungs and stomach,and is where many meridians pass. Therefore,although vocal tics are related to the five Zang,they are more related to the lungs and stomach.When Lung-stomach Qi movement in in disorders,the throat is uncomfortable,causing itching and dry cough;when the lungs and spleen are in Qi deficiency,which is unable to promote blood flow,the throat would not be nourished and nourished,prone to utter insignificant broken sound;when Qi of lungs and spleen is obnormal,dysfunctional metabolism of water and liquid would happen,with phlegm and other evil factors stagnating in the throat,causing strange sputum croaking. Wind is the first evil factor of all,which is easy to move and change. Wind can easily lead to liver wind and cause abnormal vocalization. Clinical data should be combined from four diagnosis methods,and differentiation of symptoms should be applied in treatment.
