摘 要:目的:观察白细胞分化抗原14(CD14)、白细胞介素4(IL-4)和肿瘤坏死因子α(TNF-α)在不同类型慢性根尖周病根尖周组织中的表达情况。方法:将88例样本分为3组:(1)正常对照组45例;(2)慢性根尖周脓肿组23例;(3)根尖周囊肿组20例。组织标本置于10%中性福尔马林固定液中浸泡48 h以上,制作连续组织切片。HE染色,于光学显微镜下观察各组的组织学变化;免疫荧光双染色,于荧光显微镜及共聚焦显微镜下分别观察TNF-α-CD14和IL-4-CD14双阳性细胞在根尖周组织中的表达。结果:(1)与正常对照组相比,两组慢性根尖周病组织中TNF-α-CD14及IL-4-CD14双阳性细胞密度均显著升高(P<0.05);(2)与根尖周囊肿组对比,慢性根尖周脓肿组织中TNF-α-CD14双阳性细胞密度显著升高(P<0.05);(3)与慢性根尖周脓肿组相比较,根尖周囊肿组IL-4-CD14双阳性细胞密度显著升高(P<0.05)。结论:TNF-α-CD14和IL-4-CD14双阳性细胞分别在慢性根尖周脓肿及根尖周囊肿中高表达。AIM: To observe the expression of cluser of differentiation 14(CD14), tumor necrosis factor-α(TNF-α) and interleukin-4(IL-4) in periapical tissues of patients with chronic periapical diseases, and to analyze the role of immunopathogenesis of CD14, TNF-α and IL-4 expression in human chronic periapical diseases. METHODS: A total of 88 samples were divided into 3 groups: healthy control group(n=45), chronic periapical abscess group(n=23), and periapical cyst group(n=20). All samples were fixed in 10% buffered formalin and stained with double immunofluorescence for identification of TNF-α-CD14 and IL-4-CD14 double-positive cells in periapical tissues. RESULTS: Compared with healthy control group, the densities of TNF-α-CD14 and IL-4-CD14 double-positive cells in the 2 groups of chronic periapical diseases were increased significantly(P<0.05). Compared with periapical cyst group, the density of TNF-α-CD14 double-positive cells in chronic periapical abscess was increased significantly(P<0.05). The density of IL-4-CD14 double-positive cells in periapical cyst group was significantly higher than that in chronic periapical abscess group(P<0.05). CONCLUSION: There is high expression of TNF-α-CD14 and IL-4-CD14 double-positive cells in the periapical tissues of the patients with chronic periapical abscess and perapical cyst, suggesting that the Th cytokines participate in the immune regulation of periapical diseases, which may be one of the immune mechanisms of the interaction between Th1 and Th2 cytokines.
