摘　　要：目的探讨密闭式气胸单孔胸腹腔镜手术联合三野淋巴结清扫术治疗食管癌的临床疗效。方法选取自2017年1月至2019年12月复旦大学附属浦东医院收治的130例食管癌患者为研究对象。根据随机数字表法将患者分为传统治疗组和联合治疗组,每组各65例。传统治疗组给予传统开胸手术治疗,联合治疗组给予密闭式气胸单孔胸腹腔镜手术联合三野淋巴结清扫术治疗,比较两组患者的手术情况、治疗后并发症的发生情况及生活质量评价量表(SF-36)的评分情况。结果联合治疗组患者手术时间、胸液引流时间、住院时间均明显短于传统治疗组,术中出血量、胸液引流量均明显低于传统治疗组,两组间比较,差异均有统计学意义(P<0.05)。治疗后,两组患者并发症的发生率比较,差异无统计学意义(P>0.05)。治疗后,联合治疗组患者的精神健康、情感职能、精力、健康状况、躯体疼痛及生理机能评分均明显高于传统治疗组,两组间比较,差异均有统计学意义(P<0.05)。结论密闭式气胸单孔胸腹腔镜手术联合三野淋巴结清扫术治疗食管癌的临床疗效显著,术后患者恢复快,可明显改善患者的生活质量。Objective To investigate the clinical effects of closed pneumothorax single hole thoracoscopy combined with three-field lymph node dissection in the treatment esophageal cancer.Methods A retrospective study was performed on 130 cases of patients with esophageal cancer who were admitted from January 2017 to December 2019.Patients were randomly divided into the traditional treatment group and the combination treatment group,with 65 cases in each group.Patients of the traditional treatment group were treated with traditional thoracotomy,while the combined treatment group of patients were treated with closed pneumothorax single hole thoracoscopy combined with three-field lymph node dissection.The two groups were compared in terms of surgical status,post-treatment complications and quality of life rating scale(SF-36).Results The operation time,drainage time and hospitalization time of patients in the combined treatment group were significantly shorter than those in the traditional treatment group,and the intraoperative blood loss and drainage volume of thoracic fluid were significantly lower than those in the traditional treatment group,and the differences between the two groups were statistically significant(P<0.05).Conclusion The treatment of closed pneumothorax single-hole thoracic laparoscopy combined with three-field lymphadenectomy for esophageal cancer has a significant clinical effect,which can significantly promote patient recovery and improve quality of life.
