不同热塑体膜固定技术在直肠肿瘤患者中的应用改进

摘　　要：目的探讨在直肠肿瘤患者放疗中不同热塑体膜固定技术对摆位误差的影响.方法选取本院放疗科2016年7月至2017年12月收治的腹部较窄而盆腔部较宽的直肠肿瘤患者60例.随机分为观察组和对照组,采用不同位置卡槽的固定方式,在每位患者定位、复位以及第3、5周进行CT扫描,对两组患者的摆位误差进行测量.结果观察组在X、Y、Z、Y5、Y10方向的摆位误差分别为(2.95±0.84)、(3.25±0.82)、(3.46±0.71)、(3.58±0.69)、(4.37±0.91).对照组在X、Y、Z、Y5、Y10方向的摆位误差分别为(3.09±0.75)、(3.30±0.77)、(3.47±0.67)、(4.88±0.92)、(5.61±1.12).两组在X、Y、Z方向上差异无统计学意义(F分别为1.44、0.32、0.01,P值均>0.05),在Y5、Y10方向上差异具有统计学意义(F分别为9.620、8.270,P值均<0.05).结论在直肠肿瘤患者中,根据患者腹部和盆腔部的具体情况选择合适的内外扣,使得腹部的移动空间减少,提高了摆位治疗的精确度.Objective To investigate the effect of different thermoplastic body membrane fixation techniques on the positioning error of patients with rectal cancer during radiotherapy.Methods FromJuly 2016 to December 2017,60 patients with rectal tumors with narrow abdomen and wide pelvis in our hospital radiotherapy department were selected.The patients were randomly divided into the observation group and the control group,and the fixation method of card slot at different positions was adopted.CT scan was performed at the location,reduction and the 3rd and 5th week of each patient,and the positioning error of the patients in the two groups was measured.Results The positioning errors of observation group in X,Y,Z,Y5 and YlO directions were(2.95±0.84),(3.25±0.82),(3.46±0.71),(3.58±0.69)and(4.37±0.91),respectively.The positioning errors of the control group in X,Y,Z,Y5 and YlO directions were(3.09±0.75),(3.30±0.77),(3.47±0.67),(4.88±0.92)and(5.61±1.12),respectively.There were no statistically significant differences between the two groups in the X,Y and Z directions(F was 1.44,0.32 and 0.01,all P values were>0.05),while there were statistically significant differences in the Y5 and YlO directions(F was 9.620 and 8.270,aU P values were<0.05).Conclusion In patients with rectal cancer,appropriate internal and external buttons are selected according to the specific conditions of the patient's abdomen and pelvic cavity,which reduces the abdominal moving space and improves the accuracy of positioning treatment.
