摘  要:目的探讨3D打印非共面导板复位对125I粒子植入术中穿刺准确性的影响。方法回顾性分析接受3D打印非共面导板引导下125I粒子植入治疗的25例恶性肿瘤患者,对术前导板复位及术中穿刺针道偏移情况进行分级;观察A、B、C级导板复位患者中Ⅰ、Ⅱ、Ⅲ级针分布的差异,分析导板复位等级与穿刺针等级间的相关性。结果25例共穿刺217根针。导板复位A级14例,其中Ⅰ级针105根、Ⅱ级针12根、Ⅲ级针1根;导板复位B级11例,其中Ⅰ级针71根、Ⅱ级针21根、Ⅲ级针7根;无导板复位C级。导板复位A、B级患者中,Ⅰ、Ⅱ、Ⅲ级针分布差异均有统计学意义(P均<0.01)。相关性分析结果显示,导板复位等级与穿刺针等级呈正相关(rs=0.226,P=0.010)。结论3D打印非共面导板复位与穿刺准确性相关,保证导板复位精度有助于提高125I粒子植入的穿刺准确性。Objective To explore the impact of 3D printing of non-coplanar template reduction on the puncture accuracy of 125I seeds implantation.Methods The clinical data of 25 patients with malignant tumors received implantation of 125I seeds under the guidance of 3D printing non-coplanar template were analyzed retrospectively.The preoperative template reduction(A,B,C)and intraoperative needle deviation(Ⅰ,Ⅱ,Ⅲ)were graded,the distribution of gradeⅠ,ⅡandⅢgrade needles were analyzed,and the correlation between template reduction grade and needle grade was analyzed.Results A total of 217 needles were punctured in 25 patients,14 of which had grade A template reduction,including 105 of gradeⅠneedles,12 of gradeⅡneedles,1 of gradeⅢneedle,while the other 11 had grade B template reduction,including 71 of gradeⅠneedles,21 of gradeⅡneedles and 7 of gradeⅢneedles.No grade C template reduction was observed.There were significant differences of distribution ofⅠ,ⅡandⅢneedles among patients with template reduction A and B(all P<0.01).Correlation analysis showed that was a positive correlation between template reduction grade and puncture needle grade(r s=0.226,P=0.010).Conclusion There is significant correlation between the 3D printing non-coplanar template reduction and puncture accuracy.Ensuring the accuracy of template reduction is helpful to improving puncture accuracy of 125I seeds implantation.
