清热利湿益肾解毒汤治疗子宫颈上皮内瘤变疗效及对GDF-15、Wnt-11、LRP-6水平影响的机制研究

摘　　要：目的探讨清热利湿益肾解毒汤在子宫颈上皮内瘤变患者中的疗效及对GDF-15、Wnt-11、LRP-6指标水平的影响。方法 108例子宫颈上皮内瘤变患者,按照患者入院先后顺序进行登记,随机数字表法将患者分为对照组与治疗组,每组54例。对照组患者采用伊曲康唑胶囊联合保妇康栓口服治疗,治疗组采用清热利湿益肾解毒汤药治疗,7 d为1个疗程,连续治疗10个疗程。结果①治疗后对照组与治疗组的临床总有效率分别为72.22%(39/54)、90.74%(49/54),比较差异具有统计学意义(P<0.05);②治疗前两组血清生长分化因子15(GDF-15)指标比较差异无统计学意义(P>0.05),治疗后两组数据均降低,治疗组患者GDF-15水平降低程度与对照组比较更为显著(P<0.05);③治疗后两组低密度脂蛋白受体相关蛋白-6(LRP-6)、Wnt-11水平与治疗前均降低,治疗组与对照组比较降低程度有统计学意义(P<0.05)。结论 GDF-15、Wnt-11及LRP-6指标水平的变化与子宫颈上皮内瘤变的恶化程度具有相关性,采用清热利湿益肾解毒汤剂治疗后,3个指标的水平均有降低趋势,表明患者的疾病得到有效的治疗作用。Objective To investigate the effect of Qingre Lishi Yishen Jiedu Decoction on patients with cervical intraepithelial neoplasia and the influence on GDF-15, Wnt-11 and LRP-6.Methods A total of 108 patients with cervical intraepithelial neoplasia were chosen. According to the order of admission, the patients were divided into the control group and the treatment group by random number table method, 54 cases in each group. Patients in the control group were treated with itraconazole capsules combined with Baofukang suppository oral treatment. The treatment group was treated with Qingre Lishi Yishen Jiedu Decoction. Seven days were as one course of treatment, lasting for 10 courses for continuous treatment. Results ①After treatment, the total clinical effective rates of the control group and the treatment group were 72.22%(39/54) and 90.74%(49/54), and the differences had statistical significance(P<0.05). ②There were no significant differences in serum growth differentiation factor 15(GDF-15) index between the two groups before treatment(P>0.05). After treatment, the data in both groups decreased, and the decrease in GDF-15 level in the treatment group was more significant than that in the control group(P<0.05). ③After treatment, the levels of low density lipoprotein receptor-associated protein-6(LRP-6) and Wnt-11 in the two groups were decreased before treatment, and the decreasing level had statistical significance between the treatment group and the control group(P<0.05). Conclusions The levels of GDF-15, Wnt-11 and LRP-6 are correlated with the worsening degree of cervical intraepithelial neoplasia. After treatment with Qingre Lishi Yishen Jiedu Decoction, the levels of the three indicators are decreased. The trend indicates that the disease has been effectively treated.
