摘 要:目的探讨数据系统分类标准(BI-RADS)联合超微血管成像(SMI)技术在乳腺实性结节鉴别诊断中的应用效果。方法回顾性分析2018年7月~2019年8月收治的94例乳腺结节患者临床资料。所有患者均行常规超声和SMI检查,根据二维超声结果予以BI-RADS分类,并经手术或穿刺病理活检。评估BI-RADS分类、SMI技术单独及联合诊断乳腺实性结节的诊断结果,计算诊断效能(准确度、敏感度、特异度、阳性及阴性预测值),绘制ROC曲线比较不同诊断方法效能。结果①SMI诊断乳腺结节良恶性灵敏度、阴性预测值高于BI-RADS分类,特异度、准确率、阳性预测值低于BI-RADS分类,联合检测灵敏度、特异度、准确率、阳性预测值、阴性预测值均高于BI-RADS分类及SMI单独诊断;②联合检查诊断曲线下面积(AUC)高于BI-RADS分类和SMI,SMI诊断AUC高于BI-RADS分类(P<0.05)。结论BI-RADS分类联合SMI技术在乳腺实性结节鉴别诊断中效能较高,临床应用前景可观。Objective To investigate the application effect of breast imaging reporting and data system(BI-RADS)combined with ultra-microvascular imaging(SMI)in the differential diagnosis of solid nodules in breast.Methods The clinical data of 94 patients with breast nodules admitted between July 2018 and August 2019 were retrospectively analyzed.All patients underwent routine ultrasound and SMI,and were classified according to BI-RADS according to the results of two-dimensional ultrasound,and underwent surgery or puncture biopsy.The diagnostic results of BI-RADS classification,SMI technique alone and the combination of the above two in the diagnosis of solid nodules of the breast were evaluated.The diagnostic efficacy(accuracy,sensitivity,specificity,positive and negative predictive values)was calculated.The ROC curve was plotted to compare the efficacy between different diagnostic methods.Results①The sensitivity and negative predictive value of SMI in diagnosis of benign and malignant breast nodules was higher than that of BI-RADS classification,and specificity,accuracy,positive predictive value was lower than that of BI-RADS classification.The sensitivity,specificity,accuracy,positive predictive value,negative predictive value of the combined detection was higher than that of the BI-RADS classification and SMI alone diagnosis.②The area under the curve(AUC)was higher than that of the BI-RADS classification and SMI,and the diagnosis AUC of SMI was higher than that of the BI-RADS classification(P<0.05).Conclusion The BI-RADS classification combined with SMI technology has a high efficacy in the differential diagnosis of solid nodules in breast,and its clinical application prospect is considerable.
