摘　　要：目的探讨胎儿神经学超声检查(NSG)及磁共振成像(MRI)对胎儿中枢神经系统异常的诊断价值及相互关系。方法回顾性分析同期进行NSG及MRI检查的758例胎儿中枢神经系统异常的病例,将两者结果不一致的病例与生后随访或多学科会诊结果进行比较分析。结果在758例中,NSG和MRI结果一致的有688例(91%),不一致的70例(9%)。不一致的病例中,16例(16/70,23%)MRI提供了准确信息,40例(40/70,57%)NSG提供了准确信息,1例(1/70,1%)NSG及MRI均不正确,13例(13/70,19%)患者失访。结论对于胎儿中枢神经系统异常的诊断,NSG与MRI有较好的一致性,单纯的NSG即可诊断大部分的中枢神经系统异常。对于NSG与MRI结果不一致时应将两者结合分析,互为补充,各有优势,而不能一概而论的认定MRI的准确性一定优于超声检查。Objective To evaluate the diagnostic roles and interrelation of the neurosonogram(NSG)and MRI for fetal central nervous system abnormalities.Methods 758 fetal cases with central nervous system abnormalities,who got NSG and MRI together,were analyzed retrospectively.The diagnoses of the cases,whose results of NSG were different from those of MRI,were compared with the diagnoses of newborns or the diagnoses of the multidisciplinary consultations.Results Among 758 cases,688(688/758,91%)cases′s diagnosises of NSG were consistent with those of MRI.(70/758,9%)cases′diagnosises were inconsistent.Among the inconsistent cases,16(16/70,23%)cases got the correct MRI results,and 40(40/70,57%)cases got the correct NSG results.Neither result was correct for 1 case(1/70,1%).13 cases(13/70,19%)were lost to follow-up.Conclusions There was a high level of consistency between NSG snd MRI for fetal central nervous system abnormalities.NSG can achieve the correct diagnosis independently for most abnormalities.For the cases whose results of NSG and MRI were inconsistent,the two methods should be combined and analyzed together,since each one has its own advantages.It is wrong that the accuracy of MRI is believed to be better than that of NSG in any case.
