加减星蒌承气汤对脑梗死急性期痰热腑实证患者神经功能的影响

摘　　要：目的观察加减星蒌承气汤对脑梗死急性期痰热腑实证患者神经功能的影响。方法将100例脑梗死急性期痰热腑实证患者随机分为两组各50例,两组均进行西医常规治疗,治疗组另予加减星蒌承气汤治疗。观察两组治疗前后日常生活活动能力(Barthel)、美国国立卫生研究院卒中量表(NIHSS)评分,中医证候评分,检测血液流变学指标、颈动脉内中膜厚度(IMT)、肝门静脉(PV),比较两组临床疗效。结果治疗后观察组Barthel及NIHSS评分、IMT、PV改善均优于对照组(P<0.05);血浆纤维蛋白原、血浆黏度、全血高切黏度、全血低切黏度显著低于对照组(P<0.05);中医证候评分低于对照组(P<0.05);临床疗效优于对照组(P<0.05)。结论脑梗死急性期痰热腑实证患者实施加减星蒌承气汤治疗效果显著,可利于神经功能的改善和恢复,提高总有效率,加速中医证候的恢复。Objective:To explore the effects of Modified Xinglou Chengqi Decoction on the neurological function of patients with acute cerebral infarction.Methods:100 patients were randomly divided,50 cases in each.They were treated with routine therapy and Modified Xinglou Chengqi Decoction,respectively.Some indictors were decected and compared between 2 groups including Barthel score,NIHSS score,TCM symptoms score,hemorheology indices,IMT,PV and clinical effects.Results:In the observation group,Barthel score,NIHSS score,plasma fibrinogen,plasma viscosity,whole blood high shear viscosity,whole blood low-cut viscosity,hemiplegia blood stasis score,oral angle and phlegm dampness score,dry constipation internal fire Integral,speech qi deficiency index,IMT,PV and total effective rate were better than those of the control group(P<0.05).Conclusion:The therapeutic effect of Modified Xinglou Chengqi Decoction is remarkable on patients of phlegm heat and viscera retention in acute stage of cerebral infarction.It is beneficial to the improvement and recovery of nerve function,improve the total effective rate and accelerate the recovery of TCM syndrome.
