摘 要:目的:探讨利用手机下载POCO相机软件,并应用其中的双重曝光照相技术无创、快速定位颅内病变、颅内重要血管及功能区的精确性及其应用价值。方法:首先利用传统定位方法对40例颅脑手术病人进行术前定位颅内病变、重要血管及功能区位置(对照组1);利用手机下载POCO相机软件,并应用其中的双重曝光照相技术,结合普通MRI片或CT片的三维片(冠状位、矢状位、轴位),对40例病人再次进行术前定位(实验组);最后利用MRI手术导航系统重新定位(对照组2),纠正定位偏差,指导切口设计。开颅,进一步比对验证该方法的定位效果、切除病变,随访手术效果。结果:利用手机POCO相机软件双重曝光技术定位精确性优于传统方法定位(P=0.005),同时精确性与导航系统定位相当(P=0.922);导航系统定位精确性优于传统方法定位(P=0.002);术后当天头颅CT复查,占位切除满意,周围水肿较术前无明显加重,随访患者术后肢体、语言、认知恢复良好。结论:利用手机下载POCO相机软件并利用其中的双重曝光技术定位颅内病变位置、重要血管及功能区定位准确,无创、可反复应用,简易、快速,直观可视,病患无额外费用,操作简单,普及性强,有极大的应用推广价值,尤其适用于急诊、中下规模、无手术导航系统的医院术前定位。但其在其他非颅内位置病变的定位应用方面有待进一步研究探讨。Objective:To explore the accuracy and application value of using mobile phone to download POCO camera software and using the dual-exposure photography technology,which is non-invasive,fast to locate intracranial lesions,important intracranial blood vessels and functional areas.Methods:40 cases of craniocerebral surgery patients were located preoperative intracranial lesions,important blood vessels and functional areas by traditional localization method(control group 1).Using the mobile phone to download the POCO camera software and the dual-exposure photography technology,40 patients were repositioned before surgery(experimental group) in combination with the three-dimensional MRI or CT images(coronal,sagittal and axial).Finally,the MRI surgical navigation system was used for repositioning(control group 2) to correct the positioning deviation cation and guide the incision design.Craniotomy was performed to further compare and verify of the localization effect of this method,resection of lesions,and follow-up of the operation effect.Results:The positioning accuracy of mobile phone POCO camera software was better than that of traditional positioning method(P=0.005),and the positioning accuracy was equal to that of navigation system(P=0.922).The positioning accuracy of navigation system was better than that of traditional method(P=0.002).The CT review of the head on the day after the operation showed satisfactory space-occupying resection and no significant increase in peripheral edema compared with the preoperative situation.The postoperative body,language and cognitive recovery of follow-up patients were good.Conclusion:The POCO camera software is downloaded by the mobile phone and the intracranial lesion,important vessels and functional area are located by the double exposure technology.The technology is noninvasive and can be repeatedly used,easy,fast,intuitive visual.Patients have no additional cost.It has great application value,especially suitable for emergency,middle scale,hospital preoperative localization
