摘　　要：目的:研究神经纤毛蛋白-1(neuropilin-1,NRP-1)在乳腺癌患者血清中的表达及意义。方法:选取50例乳腺癌患者,25例良性乳腺疾病患者作为研究对象。应用酶联免疫吸附试验检测血清NRP-1的表达水平,采用ROC曲线评价血清NRP-1对于乳腺癌的临床诊断价值,并以癌胚抗原(CEA)、糖类抗原-125(CA-125)、糖类抗原-153(CA-153)做为参照指标,以Youden指数作为临床诊断实验准确度评价指标,预测患者血清中NRP-1的诊断界值。分析血清NRP-1表达与乳腺癌病理学特征的关系,另将收到的乳腺癌组中25例患者的术后短期内的血清与该部分患者术前血清NRP-1比较术前及术后变化情况。结果:乳腺癌患者血清NRP-1水平高于乳腺良性疾病组(P=0.025),血清NRP-1的表达与TNM分期、肿瘤大小及有无淋巴结转移具有明显相关性(P均<0.05),而与患者的年龄、病理分子亚型及病理分级无明显相关性(P均>0.05)。术前与术后NRP-1变化无明显统计学意义(P>0.05)。结论:NRP-1有望成为早期乳腺癌的血清学诊断指标。Objective:To investigate the diagnostic value of neuropilin-1(NRP-1) in the breast cancer,and explore the association between its expression and clinicopathological characteristics of breast cancer patients.Methods:50 breast cancer patients who was newly diagnosed and untreated,and 25 patients diagnosed with benign breast disease were involved in this study.The expression of NRP-1 proteins in the serum was detected by ELISA.ROC curve was used to evaluate the clinical diagnostic value of NRP-1 for breast cancer,and carcinoembryonic antigen(CEA),carbohydrate antigen-125(CA-125),carbohydrate antigen-153(CA-153) were used as reference indexes.Youden index was used to evaluate the accuracy of clinical diagnostic experiments and to predict the diagnostic threshold of NRP-1 in serum.The relationship between the expression of serum NRP-1 and the pathological characteristics of breast cancer was analyzed.In addition,NRP-1 expression in the postoperative serum of 25 patients in breast cancer group was compared with the preoperative serum of those patients.Results:Compared with the benign disease group,the circulating NRP-1 level in breast cancer patients was significantly increased(P=0.025).The expression of circulating NRP-1 was significantly correlated with TNM stage,tumor size and lymph node status(P<0.05),but not with age,phenotype and nuclear grade(P>0.05).No statistically significant difference in NRP-1 was observed between pre-operative group and post-operative group(P>0.05).Conclusion:NRP-1 is expected to be a serological diagnostic indicator for early breast cancer.
