摘　　要：目的探讨多发性骨髓瘤患者血清中白细胞介素-6(IL-6)和白细胞介素-8(IL-8)水平与患者Mayo分层及肾功能损伤的关系。方法选择2015年1月-2018年7月在本院血液内科被确诊为多发性骨髓瘤的116例患者作为研究对象,所有患者均抽取外周血检测血清中IL-6、IL-8,对患者病情进行Mayo分层,对比不同Mayo分层患者IL-6、IL-8水平,收集患者临床资料,采用Logistics回归模型探讨患者肾脏损伤的独立危险因素。结果高危组患者IL-6、IL-8水平分别为(64.5±14.8)mg/L、(72.2±12.9)mg/L,明显高于中危、标危组患者,IL-6、IL-8与患者Mayo分层、肾功能损伤、预后均成正相关(r值分别为0.29、0.38、0.36和0.45、0.038、0.38,P<0.05);而与治疗效果均成负相关(r值分别为-0.52和-0.5,P<0.05),证实IL-6、IL-8、蛋白尿(P<0.05)均是患者急性肾功能衰竭的独立危险因素。结论多发性骨髓瘤患者IL-6、IL-8水平与患者肾功能衰竭和病情危险程度密切相关。Objective To investigate the relationship between serum levels of IL-6 and IL-8 and Mayo stratification and renal function impairment in patients with multiple myeloma.Methods From January 2015 to July 2018,116 patients with multiple myeloma were selected as the research objects.Peripheral blood was taken from all patients to detect IL-6 and IL-8 in serum.Mayo stratification was carried out to compare the levels of IL-6 and IL-8 in patients with different Mayo stratifications.The clinical data of patients was collected to explore the independent risk factors of kidney injury by Logistic regression model method.Results The levels of IL-6 and IL-8 in high-risk group were(64.5±14.8)mg/L and(72.2±12.9)mg/L respectively,which were significantly higher than those in medium-risk group and standard-risk group.IL-6 and IL-8 were positively correlated with Mayo stratification,renal function damage and prognosis(r=0.29,r=0.38,r=0.36 and r=0.45,r=0.038,r=0.38,P<0.05),while negatively correlated with therapeutic effect(r=-0.52,r=-0.5,P<0.05).It is confirmed that IL-6,IL-8 and proteinuria(P<0.05)are the independent risk factors for acute renal failure.Conclusion The levels of IL-6 and IL-8 in patients with total multiple myeloma are closely related to the degree of renal failure and disease risk.
