摘 要:目的:采用步态分析,探讨PNF技术结合低频刺激仪对脑卒中足下垂患者步行功能的影响.方法:32例脑卒中偏瘫患者随机分成观察组(n=16)和对照组(n=16).2组均接受常规康复训练,观察组增加PNF技术与足下垂刺激仪,并根据患者踝关节跖屈、外翻程度来调整电极贴片的位置及具体刺激量.治疗前和治疗8周后分别采用三维步态时空参数,FMA下肢运动功能进行评估.结果:2组患者治疗前各临床评价指标间比较均无统计学意义(P>0.05).训练8周后,对照组和观察组步长、步幅、步频、单腿支撑时间、FMA下肢运动功能较治疗前均有统计学意义(P<0.05).步态周期、双支撑相、患侧摆动相、健侧支撑相较治疗前显著减少(P<0.05),治疗后2组间比较,观察组指标步长、步幅、步频、单腿支撑时间、步态周期、双支撑相、患侧摆动相、健侧支撑相、FMA下肢运动评分均优于对照组(P<0.05).结论:PNF技术结合足下垂刺激仪能有效改善脑卒中足下垂患者的步速、步频、步行周期等时空参数,提高脑卒中后足下垂患者的步行能力及对称性.Objective:To investigate the effects of PNF combined with foot drop stimulator on gait function in patients with foot drop after stoke through gait analysis.Methods:Thirty-two stroke patients were randomly assigned into treatment group(n=16)and control group(n=16).The two groups received conventional rehabilitation therapy,while treatment group received PNF combined with foot drop stimulator in addition.According to degree of ankle joint dorsiflexion and eversion,electrode position and stimulation intensity were adjusted.Gait parameters were calculated by 3D temporal-spatial parameters gait analysis system before treatment and 8 weeks post-treatment.Result:There was no statistical significance(P>0.05) of the comparison of clinical evaluation between two groups before treatment.After 8 weeks,step length、stride length、step frequency and single limb support time in treatment group and control group were statistically significant(P<0.05).The gait parameters significantly decreased(P<0.05),such as gait cycle,double limb support phase,paretic swing phase,nonparetic stance phase.The parameters improved more significantly in treatment group than those in control group such as step length,stride length,step frequency,single limb support time,gait cycle,double limb support phase,paretic swing phase,nonparetic stance phase(P<0.05).Conclusion:PNF combined with foot drop stimulator could effectively improve stoke patientsx gait speed,step frequency,gait cycle and relevant temporal-spatial parameters,and improve gait function and gait symmetry.
