老年乳腺癌新辅助内分泌治疗的临床疗效及预后分析

摘　　要：目的探讨老年乳腺癌患者接受新辅助内分泌治疗(NET)的疗效及预后。方法选取2010年9月~2019年3月在吉林大学第一医院乳腺外科接受第3代芳香化酶抑制剂行NET的老年乳腺癌患者49例,利用检验分析不同病理特征与NET疗效的相关性;利用单因素Kaplan-Meier法及多因素COX回归分析手术治疗与预后的关系。结果孕激素受体(PR)的表达与NET临床疗效有明显相关性(P <0.05);年龄、初始肿物大小、腋窝淋巴结状态、雌激素受体(ER)及核蛋白Ki-67表达与NET的临床疗效无显著相关性(P> 0.05);是否接受手术治疗与预后无显著相关性(P> 0.05)。结论老年乳腺癌患者接受NET的疗效显著,尤其适用于ER及PR均为阳性的老年乳腺癌患者,NET后的手术治疗对老年乳腺癌患者可能不会带来明显的生存获益。Objective To investigate the clinical efficacy and prognosis of neoadjuvant endocrine therapy(NET) in elderly patients with breast cancer.Methods 49 elderly patients who received NET with aromatase inhibitors were enrolled in the First Hospital of Jilin University from September 2010 to March 2019.Chi-square test was used to analyze the correlation between pathological features and the efficacy of NET.The Kaplan-Meier univariate analysis and COX multivariate analysis were used to evaluate the correlation between surgical resection and prognosis.Results The expression of progesterone receptor(PR) was significantly correlated with the clinical efficacy of NET(P < 0.05).The clinical efficacy of NET was not significantly correlated with the age,tumor size,involvement of axillary lymph nodes,expression of estrogen receptor(ER) and Ki-67(P > 0.05).There was no significant correlation between surgical treatment and prognosis(P > 0.05).Conclusion The NET exhibits good effect,especially on cases with both ER and PR positive expression.While surgery after NET,could not bring survival benefit to older patients with breast cancer.
