摘 要:目的测算三级甲等儿科医院护士直接护理工时、间接护理工时,并构建护士合理化分层配置模型。方法于2018年5月,采用分层抽样法在我院ICU、新生儿科、普通儿科三个科室抽取不同职称的100名护士,对其直接护理工时与间接护理工时进行24 h不间断记录。根据各级护士护理工时需求量与患儿得分,构建配置模型。结果39项常规直接护理操作中,与新生儿科、普通儿科相比,ICU科护士的口鼻腔护理、吸氧、更换输液管道、药物治疗、心电监护等5项的操作时间明显更长,差异有统计学意义(P<0.05)。N0~N5级别护士的间接护理总工时由高到低依次为N0(1040.69 min)、N1(491.34 min)、N2(381.40 min)、N3(314.91 min)、N4(33.69 min)、N5(30.65 min)。分别以N0-N5护理工时需求量为应变量,以患儿得分为自变量行回归分析,可得到各级护理工时的回归方程。根据回归方程,将平均每天的护理时间定为360 min,变异系数定为0.15,出合理数学模型。结论所构建的配置模型有一定的应用价值,可试验性地应用于临床。Objective To calculate the nurses'nursing hours in the three grade A pediatric hospital and to construct a nurse rationalized stratified configuration model.Methods 100 nurses with different titles were selected,and 20 records were recorded for their nursing hours.According to the needs of nurses at all levels and the scores of patients,a configuration model was built.Results Compared with the department of pediatrics and general pediatrics,the 5 operation time of ICU department nurses such as oral cavity and nasal cavity care,oxygen inhalation,replacement of transfusion pipeline,drug treatment and ECG monitoring were significantly longer(P<0.05).The indirect nursing hours of nurses at different levels ranged from high to low in order of N1(1040.69 min),N0(491.34 min),N2(381.40 min),N3(314.91 min),N4(33.69 min),N5(30.65 min).The regression equation of nursing hours at all levels can be obtained by regression analysis with N0-N5 nursing hour demand dependent variable and patient score as independent variable.According to the regression equation,the average nursing time per day was 360 min and the coefficient of variation was 0.15,a reasonable mathematical model could be obtained.Conclusion The configuration model has certain application value,and can be applied in clinical trials.
