糖尿病治疗诱导的神经病变2例并文献复习

摘　　要：糖尿病治疗诱导的神经病变是一种独特的、急性神经性疼痛,多在强化控制血糖不久后发生,以难以忍受的疼痛为主要特点,是一种临床少见的糖尿病神经病变,易被误诊。中南大学湘雅三医院收治2例糖尿病治疗诱导的神经病变患者,现结合病例特点及相关文献报道,提出严格的血糖控制可能促使本病的发生,该病的发病机制尚不明确,其发病率远高于预想,临床医生应提高对该病的认识,通过谨慎的血糖管理来避免疾病的发生。Treatment-induced neuropathy is an distinct,acute form of neuropathic pain which often occur shortly after initiation of intensive glycemic control and is characterized by excruciating neuropathic pain. It is a rare form of diabetic neuropathy and easy to be misdiagnosed. Clinical characteristics were analyzed based on two cases of treatmentinduced neuropathy in diabetes accepted by the Third Xiangya Hospital of Central South University. It is proposed that strict glycemic control may lead to the occurrence of the disease. At Present, the pathogenesis is still unclear. Besides, the morbidity is much higher than we expected. It is important to improve the doctors awareness of this disease and avoid the the disease through carful glycemic control.
