摘  要:目的评价多排容积非减影及减影CTA在颅内小动脉瘤中的诊断价值。方法回顾性分析389例怀疑动脉瘤患者的CTA及3D DSA,以DSA及术中的结果作为金标准,计算减影前后CTA对动脉瘤诊断的敏感度、特异度及准确率。结果389例患者中,368例符合纳入标准,其中266例患者发现302个动脉瘤。减影前后CTA诊断动脉瘤整体的敏感度、特异度及准确率分别为97.0%、97.1%、97.0%和98.3%、99.0%、98.5%。对于<5 mm的动脉瘤,减影前后CTA诊断动脉瘤整体的敏感度、特异度及准确率分别为95.7%、97.1%、96.2%和97.6%、99.0%、98.1%。减影前后CTA和DSA对动脉瘤诊断的准确率无统计学差异。结论多排容积非减影及减影CTA,尤其是减影后的CTA对颅内微小动脉瘤有很高的诊断准确率,是一种高效、可靠的颅内动脉瘤检出方法。Objective To evaluate the diagnostic value of multi-row volume non-subtracted and subtracted CTA for detection of small intracranial aneurysms.Methods The CTA and 3D DSA of 389 suspected aneurysms were analyzed retrospectively.The results of DSA or intraoperative findings were used as the gold standard to calculate the sensitivity,specificity and accuracy for diagnosis of aneurysms by CTA before and after subtraction.Results Of these 389 patients,368 met the inclusion criteria and 302 aneurysms were found in 266 patients.The sensitivity,specificity and accuracy in the diagnosis of aneurysms by CTA before and after subtraction were 97.0%,97.1%,97.0%and 98.3%,99.0%,98.5%Respectively.For aneurysm less than 5 mm,the sensitivity,specificity and accuracy in the diagnosis of aneurysms by CTA before and after subtraction were 95.7%,97.1%,96.2%and 97.6%,99.0%,98.1%,respectively.There was no significant difference in the accuracy of Cl'A and DSA in the diagnosis of aneurysms before and after subtraction.Conclusion Multi-row volume non-subtracted and subtracted CTA.especially CTA after subtraction,has high diagnostic accuracy for intracranial aneurysms and is an efficient and reliable method for the detection of intracranial aneurysms.
