摘　　要：目的:探讨甲胎蛋白(AFP)、甲胎蛋白异质体3(AFP=L3)和分泌型蛋白Dickkopf-1(DKK1)联合检测对原发性肝癌的诊断价值。方法:50例原发性肝癌患者设为肝癌组,50例良性肝病患者设为良性肝病组,50例健康体检者设为对照组。检测3组对象血清AFP、AFP-L3和DKK1水平,并比较组间差异;比较肝癌组术后1个月与术前血清AFP、AFP-L3和DKK1水平差异。结果:(1)肝癌组血清AFP、AFP-L3和DKK1水平高于良性肝病组和对照组;良性肝病组血清AFP、AFP-L3水平高于对照组,差异具有统计学意义(P<0.05)。(2)肝癌组血清AFP、AFP-L3和DKK1阳性率高于良性肝病组和对照组(P<0.05)。(3)血清AFP、AFP-L3和DKK1联合检测原发性肝癌的灵敏度(90%)和准确性(88.6%)均高于单一检测,而特异度低于单一检测(P<0.05)。(4)腹腔镜肝切除术后1个月,肝癌组的血清AFP、AFP-L3和DKK1水平均降低,与术前比较差异有统计学意义(P<0.05)。结论:血清AFP、AFP-L3和DKK1联合检测在诊断原发性肝癌中的应用价值较高。Objective:To evaluate the value of combined detection of AFP,AFP-L3 and DKK1 in the diagnosis of primary liver cancer.Methods:50 patients with primary liver cancer were set as liver cancer group,50 patients with benign liver disease as benign liver disease group,and 50 healthy people as control group.The levels of AFP,AFP-L3 and DKK1 in serum of the three groups were measured,and the differences between the three groups were compared.One month after operation,the levels of AFP,AFP-L3 and DKK1 in the liver cancer group were compared with those before operation.Results:(1)AFP,AFP-L3 and DKK1 in liver cancer group were higher than those in benign liver disease group and control group,and AFP and AFP-L3 in benign liver disease group were higher than those in control group,with statistical difference(P<0.05).(2)The positive rates of AFP,AFP-L3 and DKK1 in liver cancer group were higher than those in benign liver disease group and control group(P<0.05).(3)The sensitivity(90%)and accuracy(88.6%)of combined detection of AFP,AFP-L3 and DKK1 were higher than that of single detection,but the specificity was lower than that of single detection(P<0.05).(4)One month after laparoscopic hepatectomy,the levels of AFP,AFP-L3 and DKK1 in the liver cancer group were all lower than those before operation,the difference was statistically significant(P<0.05).Conclusion:The combined detection of serum AFP,AFP-L3 and DKK1 has a high value in the diagnosis of primary liver cancer.
