血浆miR-7、miR-30e和miR-195在精神分裂症诊断中的价值分析

摘　　要：目的探究血浆微小RNA-7(miR-7)、miR-30e和miR-195对精神分裂症的诊断价值。方法选取精神分裂症患者75例(精神分裂症组)、双相情感障碍患者72例(双相情感障碍组)和同期健康体检者70例(对照组)。采用实时荧光定量PCR检测三组血浆miR-7、miR-30e和miR-195表达水平;通过受试者工作特征曲线(ROC曲线)评估血浆miR-7、miR-30e、miR-195诊断精神分裂症的价值。结果三组血浆miR-7、miR-30e、miR-195水平比较差异有统计学意义(P<0.01)。进一步两两比较发现,精神分裂症组血浆miR-7、miR-30e、miR-195水平均高于双相情感障碍组和对照组(P<0.05)。ROC曲线显示,血浆miR-7、miR-30e、miR-195诊断精神分裂症的AUC分别为0.786、0.774、0.728,三项联合诊断AUC为0.886。Z检验显示miR-7、miR-30e、miR-195之间的AUC比较差异无统计学意义(P>0.05);而三项联合诊断的AUC大于miR-7、miR-30e、miR-195单项检测的AUC(P<0.05)。诊断灵敏度、特异度:miR-7为56.0%、90.3%,miR-30e为52.0%、90.3%,miR-195为88.0%、50.0%,三项联合为94.7%、75.0%。结论精神分裂症患者血浆miR-7、miR-30e、miR-195高表达,对精神分裂症有一定的诊断价值,三项联合诊断价值更高。Objective To investigate the diagnostic value of plasma microRNA-7(miR-7),miR-30e and miR-195 for schizophrenia.Methods 75 patients with schizophrenia(schizophrenia group),72 patients with biphasic affective disorder(biphasic affective disorder group)and 70 healthy controls(control group)were selected.The expressions of miR-7,miR-30e and miR-195 in plasma of three groups were detected by real-time fluorescence quantitative PCR.The value of plasma miR-7,miR-30e,miR-195 in the diagnosis of schizophrenia was evaluated by receiver operating characteristic curve(ROC curve).Results There existed significant differences in levels of plasma miR-7,miR-30e,miR-195 among three groups(P<0.01),and levels of plasma miR-7,miR-30e,miR-195 in schizophrenia group were significantly higher than those in biphasic affective disorder group and control group(P<0.05).The ROC curve showed that the AUC of plasma miR-7,miR-30e,and miR-195 for schizophrenia were 0.786,0.774 and 0.728,respectively,and the three combined diagnosis AUC was 0.886.Z test showed that there was no significant difference in AUC among miR-7,miR-30e and miR-195(P>0.05).The AUC of three combined diagnosis was larger than those of single detections of miR-7,miR-30e and miR-195(P<0.05).Diagnostic sensitivity and specificity:miR-7 were 56.0%,90.3%;miR-30e were 52.0%,90.3%;miR-195 were 88.0%,50.0%;and the three combinations were 94.7%,75.0%.Conclusion The high expression of plasma miR-7,miR-30e and miR-195 in schizophrenics has certain diagnostic value for schizophrenia,and the combined diagnosis of the three is much more valuable.
