粤北地区肝内胆管细胞癌患者术后的预后因素分析

摘　　要：目的:分析粤北地区经手术治疗肝内胆管细胞癌(ICC)患者的预后相关因素和生存情况。方法:选取2009年1月-2016年4月于本院确诊并行手术治疗的135例ICC患者的临床资料进行回顾性分析,分析ICC患者术后生存率的影响因素。结果:术前CEA≤5 ng/mL者1、2年生存率均高于CEA>5 ng/mL者(P<0.05);术前CA199≤37 ng/mL者1、2年生存率均高于CA199>37 ng/mL者(P<0.05)。术前CA125≤35 ng/mL者1年生存率高于CA125>35 ng/mL者(P<0.05)。术前血清CEA、CA199水平均为ICC预后独立影响因素(P<0.05)。CEA(-)/CA199(-)中位生存期为18个月,CEA(+)/CA199(-)中位生存期为16个月,CEA(-)/CA199(+)中位生存期为8个月,CEA(+)/CA199(+)中位生存期为5个月。结论:术前血清CEA、CA199水平为影响粤北地区ICC患者根治术预后的独立因素,CEA、CA199均阳性的患者比至少一项阴性的患者预后更差。Objective:To analyze the prognostic factors and survival of patients with intrahepatic cholangiocarcinoma(ICC)treated surgically in northern Guangdong.Method:The clinical data of 135 patients with ICC diagnosed and operated in our hospital from January 2009 to April 2016 were retrospectively analyzed,the influencing factors of postoperative survival rate of ICC patients were analyzed.Result:The 1 and 2 years survival rate of preoperative CEA≤5 ng/mL cases were higher than those of CEA>5 ng/mL(P<0.05).The 1 and 2 years survival rate of preoperative CA199≤37 ng/mL cases were higher than those of CA199>37 ng/mL(P<0.05).1 year survival rate of preoperative CA125≤35 ng/mL cases was higher than that of CA125>35 ng/mL(P<0.05).The preoperative levels of serum CEA and CA199 were independent influencing factors for ICC prognostic(P<0.05).The median survival time was 18 months in CEA(-)/CA199(-)group,16 months in CEA(+)/CA199(-),8 months in CEA(-)/CA199(+)group and 5 months in CEA(+)/CA199(+)group.Conclusion:Preoperative serum CEA and CA199 levels are independent influencing factors for ICC patients prognostic in northern Guangdong,the prognosis of patients with positive CEA and CA199 are worse than those of patients with negative at least one.
