摘　　要：目的:探讨玻璃体腔注射雷珠单抗对玻璃体切割术治疗玻璃体积血手术效果及并发症的影响。方法:回顾性分析本院收治的86例玻璃体积血患者的临床资料,按治疗方法不同将其分为对照组(43例,49眼,玻璃体切割术)、观察组(43例,51眼,术前玻璃体腔注射雷珠单抗+玻璃体切割术),对两组手术指标、术后视力情况、术后并发症情况进行比较。结果:观察组医源性裂孔发生率、电凝使用率、眼内普通消毒气体及硅油填充率、术中大出血发生率均较对照组低(P<0.05);两组术后1、3、6、12个月最佳矫正视力均较治疗前改善(P<0.05),且观察组术后不同时期最佳矫正视力均优于对照组(P<0.001);术后随访1年,两组并发症发生情况比较,差异无统计学意义(P>0.05)。结论:玻璃体腔注射雷珠单抗有助于降低玻璃体切割术中并发症发生率,提高患者术后视力,具有良好的安全性,值得临床推广。Objective:To investigate the effect of intravitreal injection of Ranibizumab on vitrectomy for vitreous hemorrhage and complications.Method:The data of 86 patients with vitreous hemorrhage admitted to our hospital were retrospectively analyzed,according to the different treatment methods,they were divided into the control group (43 patients,49 eyes,vitrectomy) and the observation group (43 patients,51 eyes,intravitreal injection of Ranibizumab + vitrectomy).The operative index,postoperative visual recovery and postoperative complications of two groups were compared.Result:The incidence of iatrogenic hiatus,electrocoagulation,intraocular general disinfection gas and silicone oil filling and intraoperative hemorrhage in the observation group were lower than those in the control group (P<0.05).The best corrected visual acuity of two groups at 1,3,6 and 12 months after operation were better than those before treatment (P<0.05),and the best corrected visual acuity in the observation group at different periods after operation were better than those in the control group (P<0.001).One year follow-up,there was no significant difference in the incidence of complications between two groups (P>0.05).Conclusion:Intravitreal injection of Ranibizumab can reduce the incidence of complications during vitrectomy and improve the postoperative view of patients.It has good security,worthy of promotion.
