液基薄层细胞学检查联合高危型人乳头瘤病毒核酸检测在宫颈癌及癌前病变筛查中的价值

摘　　要：目的:探讨液基薄层细胞学检查(TCT)联合高危型人乳头病毒核酸检测(HR-HPV-DNA)在宫颈癌及癌前病变筛查中的临床价值。方法:选取2017年1月-2018年12月本院妇科门诊行宫颈标本病理学检查患者400例,在病检前均行TCT、HR-HPV-DNA检测,病理学结果为金标准,比较TCT、HR-HPV-DNA及联合检测的筛查价值。结果:病理学结果显示炎症、CINⅠ级、CINⅡ级、CINⅢ级、宫颈癌TCT检测阳性率(68.25%)高于HR-HPV-DNA检测(55.25%)(P<0.05);随着病理学结果分级的升高,TCT、HR-HPV-DNA检测阳性率逐渐增高,说明有明显相关性(r=0.629、0.684,P<0.05);TCT联合HR-HPV-DNA检测灵敏度高于单一检测(P<0.05),HR-HPV-DNA检测特异度高于单一TCT检测及联合检测(P<0.05);随着TCT检测宫颈病变越重,其HR-HPV感染率越高,具有明显相关性(r=0.715,P<0.05)。结论:TCT、HR-HPV-DNA检测筛查宫颈癌、癌前病变各具优势,联合检测可提高检出率及筛查价值。Objective:To explore the clinical value of TCT combined with HR-HPV-DNA in screening cervical cancer and precancerous lesions.Method:A total of 400 cases of cervical specimens from January 2017 to December 2018 in gynecological clinic of our hospital were selected for pathological examination,TCT and HR-HPV-DNA were detected before the examination,and the pathological results were the gold standard,To compare the screening value of TCT,HR-HPV-DNA and combined detection.Result:Pathological results showed inflammation,CINⅠ,CINⅡ,CINⅢand cervical cancer,the positive rate of TCT was 68.25%higher than that of HR-HPV-DNA(55.25%)(P<0.05).The positive rates of TCT and HR-HPV-DNA were gradually increased with the increase of pathological grade(r=0.629,0.684,P<0.05).The sensitivity of TCT combined with HR-HPV-DNA detection was higher than that of single detection(P<0.05),and the specificity of HR-HPV-DNA detection was higher than that of single TCT detection and combined detection(P<0.05).The more serious the cervical lesions detected by TCT,the higher the infection rate of HR-HPV,which has a significant correlation(r=0.715,P<0.05).Conclusion:TCT and HR-HPV-DNA detection have advantages in screening cervical cancer and precancerous lesions,combined detection can improve the detection rate and screening value.
