通腑活血方对术后早期炎症性肠梗阻患者的疗效及血清炎症指标的影响

摘　　要：目的探讨通腑活血方对术后早期炎症性肠梗阻(EPISBO)患者临床疗效及血清炎症指标的影响。方法将118例EPISBO患者随机分为对照组和观察组,每组59例。对照组在常规治疗基础上给予生长抑素治疗,观察组在对照组基础上给予通腑活血方口服及灌肠治疗。比较两组治疗后腹部症状缓解时间、肠鸣音恢复时间、首次排气时间、首次排便时间及临床疗效,比较两组患者治疗前后血清白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)和C反应蛋白(CRP)水平。结果观察组治疗后腹部症状缓解时间、肠鸣音恢复时间、首次排气和排便时间均短于对照组,临床总有效率高于对照组(均P<0.05)。治疗后两组的血清IL-6、TNF-α和CRP水平均低于治疗前,且观察组低于对照组(均P<0.05)。结论通腑活血方口服和灌肠辅助治疗EPISBO能明显缓解患者的临床症状,降低血清IL-6、TNF-α和CRP水平。Objective To explore the clinical efficacy of Tongfu Huoxue Recipe for patients with early postoperative inflammatory small bowel obstruction(EPISBO)and its impact on serum inflammatory indicators.Methods A total of 118 patients with EPISBO were randomly divided into control group and observation group,with 59 cases in each group.The control group was treated with somatostatin on the basis of routine treatment,based on which the observation group was given oral and enema medications using Tongfu Huoxue Recipe.The abdominal symptom remission time,bowel sound recovery time,first exhaust time,first defecation time,and clinical efficacy after treatment were compared between the two groups,and serum levels of interleukin 6(IL-6),tumor necrosis factorα(TNF-α)and C-reactive protein(CRP)before and after treatment were compared between the two groups.Results After treatment,the observation group had shorter abdominal symptom remission time,bowel sound recovery time,first exhaust time and defecation time,and a higher total clinical effective rate in comparison with the control group(all P<0.05).After treatment,serum levels of IL-6,TNF-αand CRP were lower in both groups as compared with the levels before treatment,and the levels in the observation group were lower than those in the control group(all P<0.05).Conclusion Oral and enema medications using Tongfu Huoxue Recipe,an adjuvant therapy for EPISBO,can significantly ameliorate patients′clinical symptoms and decrease serum IL-6,TNF-αand CRP levels.
