摘　　要：目的:研究脊髓小脑共济失调3型(SCA3)患者脑脊液及血浆中γ-氨基丁酸(GABA)和谷氨酸(Glu)的水平,探讨二者在SCA3发生中的作用及临床意义。方法:使用超高效液相色谱-质谱联用法检测24例SCA3患者及9例正常对照脑脊液及血浆中GABA、Glu水平。对24例SCA3患者行国际共济失调评定量表(ICARS)评分。分析SCA3患者GABA和Glu水平与年龄、CAG重复数及ICARS评分的关系。结果:与正常对照比较,SCA3患者脑脊液与血浆中GABA水平较低,Glu水平较高(P<0.05)。SCA3患者脑脊液中GABA水平与ICARS评分呈负相关(r=-0.412,P<0.05),SCA3患者脑脊液与血浆中Glu与CAG重复数呈正相关(r=0.414、0.485,P<0.05)。结论:脑脊液中GABA水平可反映SCA3患者病情严重程度。Aim:To quantify the neurotransmitterγ-amino-butylic acid(GABA)and glutamate levels in cerebrospinal fluid(CSF)and plasma of spinocerebellar ataxia type 3(SCA3)patients and investigate the role of the neurotransmitters in pathogenesis and treatment of SCA3.Methods:CSF and blood samples were collected from 24 SCA3 patients and 9 normal controls.The ultraperformance liquid chromatography tandem mass spectrometry was used to quantify the levels of GABA and glutamate in collected samples.The ICARS was scored in the 24 SCA3 patients.The relationship between the GABA and glutamate levels in CSF and plasma of the SCA3 patients with age,CAG repeats and ICARS scores was analyzed.Results:Compared with normal controls,the level of GABA was significantly lower while the level of glutamate was significantly higher in CSF and plasma of SCA3 patients(P<0.05).The GABA level in CSF of SCA3 patients has a negative correlation with ICARS scores(r=-0.412,P<0.05),the level of glutamate in CSF and plasma of SCA3 patients has a positive correlation with CAG repeats(r=0.414,0.485,P<0.05).Conclusion:The level of GABA in CSF of SCA3 patients could reflect the severity of SCA3.
