背俞指针疗法联合半夏泻心汤治疗寒热错杂证功能性消化不良疗效及对胃肠激素水平的影响

摘　　要：目的观察背俞指针疗法联合半夏泻心汤口服治疗寒热错杂证功能性消化不良患者的临床疗效及对胃动素(MTL)、胃泌素(GAS)、胆囊收缩素(CCK)水平的影响。方法选择2017年1月—2018年10月在柳州市中医医院脾胃病科诊治的72例寒热错杂证功能性消化不良患者,按照随机数字表随机分为治疗组和对照组各36例,对照组患者给予多潘立酮片口服治疗,治疗组患者给予背俞指针疗法联合半夏泻心汤口服治疗,2组患者均治疗3周。观察2组患者治疗前及治疗3周后中医症状积分和血清MTL、血浆GAS、血浆CCK水平变化,统计2组患者治疗3周后临床疗效及治疗期间相关不良反应发生情况。结果与治疗前比较,2组患者治疗后胃脘痞满、口干苦、纳呆、便溏、嘈杂积分均明显降低(P均<0.05);治疗后比较,治疗组胃脘痞满、口干苦、纳呆、便溏、嘈杂积分均明显低于对照组(P均<0.05)。与治疗前比较,2组患者治疗后MTL水平均明显升高(P均<0.05),GAS和CCK水平均明显降低(P均<0.05);治疗后比较,治疗组MTL水平明显高于对照组(P<0.05),GAS和CCK水平均明显低于对照组(P<0.05)。治疗组和对照组临床总有效率分别为94.4%(34/36)和72.2%(26/36),治疗组临床总有效率明显高于对照组(P<0.05)。2组患者治疗期间均未出现明显不良反应。结论背俞指针疗法联合半夏泻心汤口服治疗寒热错杂证功能性消化不良患者疗效肯定、安全,其作用机制可能与调节胃肠激素水平相关。Objective It is to observe the clinical efficacy of finger pressure therapy at back-shu point combined with Banxia Xiexin Decoction on functional dyspepsia with syndrome of mixed cold and heat and its effect on the levels of motilin(MTL),gastrin(GAS)and cholecystokinin(CCK).Methods 72 patients with functional dyspepsia with syndrome of mixed cold and heat who were diagnosed and treated in the department of spleen and gastroenterology of Liuzhou Hospital of Traditional Chinese Medicine Hospital from January 2017 to October 2018 were selected and randomly divided into a treatment group and a control group according to the random number table,36 cases in each group.The patients in the control group were given oral domperidone tablets.The patients in the observation group were given finger pressure therapy at back-shu point combined with oral Banxia Xiexin Decoction.Both groups were treated for 3 weeks.The changes of TCM symptom scores and the levels serum MTL,plasma GAS,and plasma CCK levels before and after 3 weeks of treatment were observed in the two groups.The clinical curative effect after 3 weeks of treatment was assessed and the occurrence of correlated side effects during the treatment was recorded in the two groups.Results Compared with those before treatment,the scores of stomach fullness,dry mouth and bitter taste,poor appetite,loose stools,and heartburn in the two groups were significantly reduced after treatment(P<0.05),the scores of stomach fullness,dry mouth and bitter taste,poor appetite,loose stools,and heartburn after treatment in the treatment group were significantly lower than those in the control group(P<0.05).Compared with that before treatment,the levels of MTL in the two groups were increased significantly after treatment(P<0.05),and the levels of GAS and CCK were significantly decreased(P<0.05).After treatment,the MTL level in the treatment group was significantly higher and the levels of GAS and CCK were significantly lower than those in the control group(P<0.05).The total clinical effecti
