摘  要:目的:分析佳木斯地区青年隐源性卒中患者的cTCD检查结果,为青年CS患者的病因诊断提供可靠的理论依据。方法:连续性纳入90例确诊为青年隐源性卒中患者,对其进行脑血管病危险因素筛查。对所有入组患者通过经胸超声心动图(TTE)及对比增强经颅多普勒超声(cTCD)行PFO筛查,分析患者TTE、cTCD对比结果,危险因素评估,以及影像学分布特征。结果:经胸超声心动图检查的卵圆孔未闭有40例,检出率为44%;而对比增强经颅多普勒超声检查的卵圆孔未闭有56例,检出率为62%,其敏感性明显高于经胸超声心动图。且差异有统计学意义(P<0.05)。而PFO相关性青年隐源性卒中与非PFO相关性青年隐源性卒中患者在其病灶分布的部位、数量之间的差异无统计学意义。结论:cTCD对青年隐源性卒中患者卵圆孔未闭检出的敏感性较高,可用于临床青年隐源性卒中患者PFO的首选筛查手段,具有较高的临床应用价值。Objective:To analyze the results of comparative enhanced transcranial doppler ultrasound(cTCD)in young patients with cryptogenic stroke in jiamusi district,and to provide a reliable theoretical basis for the etiological diagnosis of cryptogenic stroke in young patients.Methods:A total of 90 young patients diagnosed with cryptogenic stroke were included in the study.The risk factors of cerebrovascular disease(PFO)were screened by transthoracic echocardiography(TTE)and cTCD,and the results of TTE and cTCD,risk factors assessment of cerebrovascular disease and imaging distribution characteristics of lesions were compared and analyzed.Results:Forty cases of patent foramen ovale were detected by thoracic echocardiography,the detection rate was 44%(40/90).Contrastively enhanced transcranial doppler ultrasonography detected 56 patients with patent foramen ovale,with a detection rate of 62%(52/90).The sensitivity was significantly higher than that of transthoracic echocardiography,and the difference was statistically significant(P<0.05).However,the location and number of apoplexy lesions were not significantly affected by the young cryptic stroke associated with patent foramen ovale(P<0.05).Conclusion:Contrastively enhanced transcranial doppler ultrasonography is highly sensitive to the detection of patent foramen ovale in young patients with cryptogenic stroke,which can be used as the preferred screening method for young patients with cryptogenic stroke and has a high clinical application value.
