头穴调神抑痛针法联合脑运动区皮质电刺激对脑梗死患者中枢性疼痛的影响

摘　　要：目的:探讨头穴调神抑痛针法联合脑运动区皮质电刺激治疗对脑梗死患者中枢性疼痛(CP)的作用。方法:选取82例脑梗死后CP患者,随机分为观察组和对照组,各41例。2组均接受常规治疗,对照组在常规治疗基础上给予头穴调神抑痛针法治疗,观察组在对照组基础上配合脑运动区皮质电刺激治疗,均治疗4周。比较2组疼痛视觉模拟评分(VAS)、患者运动功能及日常生活能力,比较治疗期间不良反应发生情况。结果:治疗后观察组与对照组VAS评分[(2.71±0.65)vs(3.82±0.93)分]差异有统计学意义(P<0.05);观察组与对照组总有效率比较(92.68%vs 75.61%)差异有统计学意义(P<0.05);治疗后与对照组比较,观察组上肢、下肢运动功能评分、BI评分分别[(26.45±4.03)vs(22.79±3.52)分]、[(20.31±2.90)vs(16.92±2.52)分]、[(65.43±7.20)vs(52.67±7.15)分]高于对照组;治疗期间对照组不良反应发生率(9.76%)与观察组(12.20%)比较,差异无统计学意义(P>0.05)。结论:头穴调神抑痛针法联合脑运动区皮质电刺激可显著缓解脑梗死后CP患者疼痛症状,有助于改善肢体运动功能和日常生活能力,且安全可靠。Objective:To investigate the effect of scalp acupuncture and relieving pain and acupuncture combined with cortical electrical stimulation on central pain(CP)in patients with cerebral infarction.Methods:Eighty-two patients with CP after cerebral infarction were selected and randomly divided into observation group and control group,each with 41 cases.Both groups received conventional treatment.The control group was given scalp acupuncture and analgesic acupuncture on the basis of conventional treatment.The observation group was treated with on the basis of the control group.Both were treated for 4 weeks.The visual analogue scale of pain(VAS),motor function and daily living ability of the two groups were compared,and the incidence of adverse reactions during treatment was compared.Results:After treatment,the VAS score of the observation group(2.71±0.65)was lower than that of the control group(3.82±0.93)(P<0.05);the total effective rate of the observation group(92.68%)was higher than that of the control group(75.61%)(P<0.05);After treatment,the upper and lower limb motor function scores and BI scores of the observation group were(26.45±4.03)points,(20.31±2.90)points,and(65.43±7.20)points,which were higher than the control group s(22.79±3.52)points,(16.92±2.52)points,(52.67±7.15)points(P<0.05);the incidence of adverse reactions in the control group(9.76%)during the treatment period was not significantly different from that in the observation group(12.20%)(P>0.05).Conclusion:The scalp acupuncture and pain relieving acupuncture method combined with can significantly relieve the pain symptoms of CP patients after cerebral infarction,and it can help improve limb movement function and daily life ability,and it is safe and reliable.
