关节镜下锚钉固定在青少年膝关节髁间棘骨折治疗中的应用

摘　　要：目的研究关节镜下锚钉固定在青少年膝关节髁间棘骨折治疗中的临床疗效。方法选取2013年2月~2018年2月我院收治的35例青少年膝关节髁间棘骨折患者,复位后,在关节镜下使用锚钉固定,观察手术前后患者Lysholm膝关节评分、IKDC2000主观膝关节评分、X线片骨折愈合时间。结果平均愈合时间(8±2.1)周,末次随访,患者的Lysholm膝关节评分(95.66±2.26)分、IKDC2000主观膝关节评分(80.54±5.26)分,均显著高于患者手术前Lysholm膝关节评分(35.62±3.46)分、IKDC2000主观膝关节评分(44.07±1.68)分,差异均有统计学意义(P<0.05)。结论青少年膝关节髁间棘骨折后采用关节镜下锚钉固定进行治疗,骨折端复位容易、损伤小、固定牢靠且内固定物无需进行二次手术取出,不损伤骨骺,术后可进行有效膝关节功能的功能锻炼,具有较高的临床疗效,可广泛推广。Objective To study the clinical curative effects of arthroscopic anchor fixation in treatment of adolescents with intercondylar eminence fracture of knee joint.Methods 35 adolescent patients with intercondylar eminence fracture of knee joint who were admitted and treated in our hospital from February 2013 to February 2018 were selected.After reduction,they were given arthroscopic anchor fixation.The Lysholm knee score,IKDC2000 subjective knee score,and x-ray fracture healing time were observed before and after surgery.Results The average healing time was(8±2.1)w.In the last return visit,the Lysholm knee score and IKDC2000 subjective knee score of patients were respectively(95.66±2.26)and(80.54±5.26),both significantly higher than(35.62±3.46)and(44.07±1.68)before surgery.The differences were statistically significant(P<0.05).Conclusion Arthroscopic anchor fixation in treatment of adolescents with intercondylar eminence fracture of knee joint has easy reduction of fractures,small injuries and firm fixation.Moreover,the internal fixator does not need to be removed by a second operation and does not damage epiphysis.Effective functional exercise of knee joint function can be performed after operation.It has higher clinical curative effects,which can be widely promoted.
