摘　　要：目的探讨齿状核平均峰度(MK)值在帕金森病(PD)运动障碍分型中的价值。方法选取PD患者76例和健康体检的中老年人39例(HC组),对PD患者进行统一PD评估量表评分,根据评分将PD患者分为震颤为主型(TD组)29例、姿势不稳步态障碍为主型(PIGD组)37例和不确定型10例。所有受试者均行结构磁共振成像检查和磁共振扩散峰度成像检查,在MK参数图上记录3组齿状核MK值并进行比较。采用Pearson相关分析PD患者齿状核MK值与震颤评分和姿势不稳步态障碍评分的相关性。结果TD组齿状核MK值较PIGD组和HC组均升高,差异均有统计学意义(均P<0.05),PIGD组与HC组齿状核MK值比较差异无统计学意义(P>0.05)。PD患者齿状核MK值与震颤评分呈正相关(r=0.407,P<0.05),与姿势不稳步态障碍评分无相关性(r=-0.061,P>0.05)。结论齿状核MK值有助于PD运动障碍分型,并可评估震颤严重程度。Objective To assess the value of mean kurtosis(MK)of dentate nucleus in the classification of movement disorders in Parkinson's disease(PD).Methods Seventy six PD patients and 39 healthy middle-aged and elderly subjects(HC group)were enrolled in this study.According to the unified PD rating scale assessment scores,there were 29 patients with tremor dominant(TD group),37 patients with postural instability gait difficulty(PIGD group)and 10 indeterminate patients.All the subjects underwent structural MRI and diffusion kurtosis imaging(DKI)examination.The values of MK in three groups of dentate nuclei were recorded on the MK parameter map and compared.Pearson analysis was used to analyze the correlation of MK value of dentate nucleus with tremor score and postural instability gait difficulty scores in PD patients.Results The MK value of dentate nucleus in TD group was higher than that in PIGD group and HC group(both P<0.05),while there was no statistically significant difference between PIGD group and HC group(P>0.05).The MK value of dentate nucleus was positively correlated with tremor scores(r=0.407,P<0.05),but not correlated with postural instability gait difficulty scores(r=-0.061,P>0.05).Conclusion The MK value of dentate nucleus may assist the classification of movement disorders in PD patients,and may be used to assess the tremor severity.
