摘　　要：目的:探究不同手术方式对青光眼合并白内障患者视力、眼压、散光度及并发症的影响。方法:选择2018年2月-2019年2月于笔者所在医院行手术治疗的青光眼合并白内障患者84例为研究对象,采用随机数字表法分成常规组及试验组,各42例。常规组采用白内障超声乳化术及小梁切除术治疗,试验组采用白内障超声乳化术及房角分离术治疗,均持续随访6个月,对比两组视力、眼压、前房角度、前房深度、散光度和并发症情况。结果:试验组与常规组视力提升率分别为83.33%、78.57,差异无统计学意义(P>0.05);试验组术后前房角度大于常规组,散光度低于常规组,差异均有统计学意义(P<0.05);试验组与常规组并发症发生率分别为4.76%、19.05%,差异有统计学意义(P<0.05)。结论:针对青光眼合并白内障患者采用白内障超声乳化术及房角分离术治疗可增大前房角度,降低散光度和并发症发生率,值得在临床中大力推广。Objective:To explore the effects of different surgical modes on visual acuity,intraocular pressure,astigmatic degree and complications of patients with glaucoma and cataract.Method:A total of 84 patients with glaucoma and cataract who underwent surgical treatment in our hospital from February 2018 to February 2019 were selected as the subjects of the study.According to random number table method,patients were divided into the routine group and the test group,42 cases in each group.The phacoemulsification and trabeculectomy were performed in the routine group,and the phacoemulsification and goniosynechialysis were performed in the test group,and all patients were followed up for 6 months.The visual acuity,intraocular pressure,anterior chamber angle,anterior chamber depth,astigmatic degree and complications were compared between the two groups.Result:The improvement rate of visual acuity in the test group and the routine group were 83.33%and 78.57%,respectively,and the difference was not statistically significant(P>0.05).After surgery,the anterior chamber angle in the test group was larger than that of the routine group,and astigmatic degree was lower than that of the routine group,and the differences were statistically significant(P<0.05).The complications rate of the test group and the routine group were 4.76%and 19.05%,respectively,and the difference was statistically significant(P<0.05).Conclusion:Phacoemulsification and goniosynechialysis for patients with glaucoma and cataract can increase the anterior chamber angle and reduce the astigmatic degree and complications rate,which is worth promoting in clinic.
