摘　　要：目的了解云南省脑血管病死亡人群的流行病学特征和危害程度,为政府决策提供科学依据。方法利用2014~2018年云南省死因监测脑血管病死亡资料,描述性分析居民脑血管病的死因顺位、死因构成、死亡率变化及寿命损失。结果5年间脑血管病连续位居云南省居民第2位死因,占总死亡的18.77%,死亡率呈逐年上升趋势,5年平均粗死亡率为121.79/10万,标化死亡率为130.09/10万,男性死亡率(135.29/10万)高于女性(107.36/10万)(P<0.01)。随着年龄的增长,居民脑血管病死亡率逐渐增加,尤其是60岁以后迅速上升,占脑血管病总死亡的82.33%。居民因脑血管病死亡造成的PYLL为1835286人年,PYLLR为7.74‰,AYLL为6.35年/人,PYLL和PYLLR逐年上升,5年平均PYLL和PYLLR男女之比分别为1.39和1.30。结论脑血管病是云南省居民的主要死因,死亡率呈上升趋势,其对居民的寿命损失亦逐年增高且对男性寿命的危害程度大于女性。Objective To study the epidemiological characteristics of death caused by cerebrovascular disease,so as to provide scientific support for the government’s decision-making.Methods Based on the data of death due to cerebrovascular disease from death cause surveillance in Yunnan province from 2014 to 2018,descriptive analysis was used to analyze and calculate cause of death sequence,cause of death composition,mortality and life loss of cerebrovascular disease in residents.Results The cerebrovascular disease was the second leading cause of death from 2014 to 2018 in Yunan province,accounting for 18.77%of total death.The mortality rose year by year.The average crude death rate was 121.79/10^5,and male(135.29/10^5)was much higher than female(107.36/10^5)(P<0.01).The standardized death rate was 130.09/10^5.The mortality went up with age,especially for aged above 60 years old,whose mortality rose dramatically and the death number accounted for 82.33%.During 2014 to 2018,the total potential years of life lost(PYLL),potential years of life lost rate(PYLLR)and average years of life lost(AYLL)were 1835286 person years,7.74‰and 6.35 years per person respectively.In general,the PYLL and PYLLR also rose year by year and those of male were 1.39 and 1.30 times higher than those of female respectively.Conclusion The cerebrovascular disease is the main cause of death in Yunnan province and the mortality rises year by year.The cerebrovascular disease increases the life loss of residents year by year,and the harm degree of male life is greater than that of female.
