青少年代谢综合征血清载脂蛋白B与载脂蛋白A-1比值的临床意义

摘　　要：目的探讨载脂蛋白B与载脂蛋白A-1比值(ApoB/A-1)与青少年代谢综合征(MS)的关系。方法选取2017年1月-2018年11月在本院诊治的119例肥胖症患儿为研究对象,依据肥胖症相关诊断标准分为单纯性肥胖组64例和代谢综合征组55例,另选取健康儿童60例为对照组,进行体质量指数、肝功能、血脂、空腹血糖、胰岛素和胰岛素抵抗(HOMA-IR)稳态模型评估。结果 MS患儿显示出更高的转氨酶、TG、TC、LDL-C、ApoB/A-1、空腹胰岛素和HOMA-IR水平,但HDL-C低于单纯性肥胖(OB)组和对照组(P<0.05)。ApoB/A-1与ALT、AST、TG、TC和LDL-C的水平呈正相关(r值分别为0.225、0.268、0.354、0.493、0.585,P<0.01),与HDL-C水平呈负相关(r=-0.179,P<0.05)。受试者工作特征曲线分析表明,ApoB/A-1比值及TG联合ApoB/A-1比值曲线下面积(AUC)分别为0.803和0.916。结论 ApoB/A-1比值可以作为儿童、青少年MS较为简便、有效的参考指标,可能成为MS预防、诊治过程的重要依据。Objective To investigate the relationship of serum ApoB and ApoB/A-1 ratio in the adolescents with metabolic syndrome(MS).Methods The enrolled participants were 119 cases of obesity adolescents who received inpatient treatment in our hospital from January 2017 to November 2018,including 64 with obesity and 55 with metabolic syndrome based on the diagnostic criteria,and 60 healthy adolescents were enrolled in the control group.Body mass index(BMI),fasting glucose,insulin,lipids,and homeostatic model assessment for insulin resistance(HOMA-IR) and hepatic transaminases were evaluated.Results Adolescents with MS displayed higher ALT,AST,TG,TC,LDL-C,ApoB/A-1,fasting insulin and HOMA-IR levels,but lower HDL-C than the single obesity group and control group(P<0.05).ApoB/A-1 ratio was positively correlated with ALT,AST,TG,TC and LDL-C(r = 0.225,r = 0.268,r = 0.354,r = 0.493,r = 0.585,P<0.01).ApoB/A-1 ratio was negatively correlated with HDL-C(r =-0.179,P<0.05).Receiver operating characteristic analysis indicated that the area under the curve in MS subjects for ApoB/A-1 ratio and TG combined with ApoB/A-1 ratio were 0.803 and 0.916.Conclusion In our study,the serum ApoB/A-1 ratio in MS adolescents is closely associated with the disease,which can be used as a marker for the diagnosis of MS.
