结直肠癌根治术中腹腔灌注榄香烯的安全性分析及其对免疫功能的影响

摘　　要：目的观察结直肠癌根治术中腹腔灌注榄香烯的安全性及其对机体免疫状态的影响。方法选取60例结直肠癌患者,随机分为对照组和试验组,分别接受根治性手术,在关腹前对照组腹腔灌注温生理盐水,试验组腹腔灌注榄香烯,比较两组术后首次排气时间、首次进流食时间以及并发症发生率,比较两组术前和术后血常规指标、肝肾功能指标、CD3^+T细胞、CD4^+/CD8^+T细胞、NK细胞、IL-2、IL-10、TNF-α水平。结果两组术后首次排气时间、术后首次进流食时间以及并发症的发生率比较差异无统计学意义,两组术后白细胞、血红蛋白、肝肾功能等指标比较差异无统计学意义。术后7 d,试验组CD4^+/CD8^+T细胞、NK细胞、IL-2水平高于对照组,而IL-10、TNF-α水平低于对照组(P均<0.05)。结论结直肠癌根治术中腹腔灌注榄香烯具有很好的安全性,还能从细胞和体液免疫两个方面改善机体术后的免疫功能。Objective To analyze the safety of intraoperative peritoneal lavage of elemene during radical resection of rectal carcinoma and its effect on immune function of patients with colorectal cancer.Methods Sixty patients with colorectal cancer were randomly divided into the control group and experiment group.All patients were treated with radical resection;patients in the control group were injected with warm normal saline,while patients in the experiment group were treated with elemene intraoperative peritoneal lavage before abdominal closure.The first time of exhaust,the first time of feeding,and the incidence of complications between the two groups were compared,and the changes of blood routine,liver and kidney function,CD3^+T cells,CD4^+/CD8^+T cells,NK,IL-2,IL-10,TNF-α,and other indexes before and after operation were compared.Results After operation,there were no significant differences in the first time of exhaust,the first time of feeding,the incidence of complications,the indicators of white blood cell,hemoglobin,or liver and kidney functions between the two groups(all P>0.05).The levels of CD4^+/CD8^+T cells,NK,and IL-2 were higher in the experiment group than in the control group,while the levels of IL-10 and TNF-αwere lower than those of the control group on day 7 after operation,with statistically significant difference between the two groups(all P<0.05).Conclusion The safety of intraoperative peritoneal lavage of elemene during radical resection of colorectal cancer is good,and this method can improve the postoperative immune function from both cellular and humoral immunity.
