摘　　要：老年性认知障碍是指发生在老年期的认知功能障碍,属于中医学健忘、痴呆、呆病范畴,张学文国医大师突破传统理论,提出“脑当为脏论”,基于脑脏理论和脑髓生理病理特点的认识,总结出老年性认知障碍的形成路径是肾虚不能生髓充脑是根本原因;脏腑失调、气血亏虚、脑髓失养是必要条件;脑髓空虚、痰瘀阻滞脑髓是关键环节;脑髓失充失养、神明失用是最终病理结果。张学文国医大师提出清肝和血、化瘀通络的治疗大法,自拟清脑通络汤,治疗老年性认知障碍患者取得了较好临床疗效,希望这些研究对指导老年性认知障碍患者的临床治疗有一定的借鉴意义。Senile cognitive impairment refers to cognitive dysfunction that occurs in old age,and belongs to the cate-gories of forgetfulness and dementia in traditional Chinese medicine.Zhang Xuewen,a master of Chinese medicine,broke through the traditional theory and put forward the theory of“brain as viscera”.Based on the theory of“brain is the zang of vitality”and the understanding of the physiological and pathological characteristics of brain marrow,the author concludes that the formation of senile cognitive impairment is that kidney deficiency can not generate marrow and fill the brain;The essential conditions are the disorder of zang fu organs,deficiency of qi and blood,and the loss of nourishment of brain and marrow;the key lies in the deficiency of brain and marrow and the stagnation of phlegm and blood stasis;the final pathological results are the loss of nourishment of brain and marrow and mental abnormality.TCM Master Zhang Xuewen put forward the treatment of clearing liver-fire,nourishing blood and promoting blood circulation,dispersing blood stasis and dredging collateral,and made Qingnao Tongluo Decoction.It is hoped that these studies will have certain reference significance in guiding the clinical treatment of patients with senile cognitive impairment.
