龙葵葶苈汤联合顺铂化疗对肺癌大量胸腔积液患者的治疗效果及免疫指标的影响

摘　　要：目的分析龙葵葶苈汤联合顺铂化疗对肺癌大量胸腔积液患者的治疗效果及免疫指标的影响。方法选取2015年6月至2016年6月南通市肿瘤医院中西医结合科收治的70例肺癌大量胸腔积液患者为研究对象,根据治疗方案不同分为对照组和观察组,每组35例。其中,对照组予以顺铂化疗,观察组予以龙葵葶苈汤联合顺铂化疗,治疗3周。治疗后比较两组患者的治疗效果、生活质量改善效果、免疫指标(CD3、CD4、CD8)、不良反应(食欲下降、恶心呕吐、发热)以及生存情况。结果观察组的总有效率高于对照组[88.6%(31/35)比51.4%(18/35)](P<0.05)。观察组的生活质量改善率高于对照组[82.9%(29/35)比48.6%(17/35)](P<0.05)。治疗后,两组患者的CD3、CD4、CD8水平高于治疗前(P<0.05),且观察组高于对照组[(59.4±3.8)%比(52.5±4.2)%,(47.0±3.6)%比(42.9±4.4)%,(39.7±3.1)%比(35.5±3.8)%](P<0.05),CD3、CD4、CD8的组间、时点间、组间和时点间交互作用比较差异有统计学意义(P<0.05)。观察组总不良反应发生率低于对照组[11.4%(4/35)比40.0%(14/35](P<0.05)。观察组中位无进展生存期长于对照组(11个月比9个月)(P<0.05)。结论龙葵葶苈汤联合顺铂化疗对肺癌大量胸腔积液患者的治疗效果显著,可明显改善患者的生活质量与免疫功能,延长患者的生存期,且不良反应发生率低。Objective To analyze the effect of Longkui Tingli decoction combined with cisplatin chemotherapy on lung cancer patients with massive pleural effusion and its influence on the immune indexes.Methods From Jun.2015 to Jun.2016,70 patients with massive pleural effusion of lung cancer from the Department of Integrated Traditional Chinese and Western Medicine of Nantong Tumor Hospital were included in the study.According to the different treatment plans,they were divided into a control group and an observation group,35 cases each.The control group was treated with cisplatin chemotherapy,while the observation group was treated with Longkui Tingli decoction combined with cisplatin chemotherapy,for 3 weeks.After treatment,the therapeutic effect,quality of life improvement,immune indexes(CD3,CD4,CD8),adverse reactions(appetite decline,nausea and vomiting,fever)and survival of the two groups were compared.Results The total effective rate in the observation group was higher than that in the control group[88.6%(31/35)vs 51.4%(18/35)](P<0.05).The improvement rate of quality of life in the observation group was higher than that in the control group[82.9%(29/35)vs 48.6%(17/35)](P<0.05).After treatment,the levels of CD3,CD4 and CD8 in the two groups were higher than those before treatment(P<0.05),and the observation group higher than the control group[(59.4±3.8)%vs(52.5±4.2)%,(47.0±3.6)%vs(42.9±4.4)%,(39.7±3.1)%vs(35.5±3.8)%](P<0.05),and the differences were statistically significant between groups,time points,interactions of group and time point(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group[11.4%(4/35)vs40.0%(14/35)(P<0.05).The median progression free survival time of the observation group was longer than that of the control group(11 months vs 9 months)(P<0.05).Conclusion Longkui Ting Li decoction combined with cisplatin chemotherapy has a significant effect on the lung cancer patients with massive pleural effusion,which can significantly improve the quality o
