摘　　要：目的:探讨抗肾小球基底膜(GBM)抗体相关疾病的临床特征,加深对该组疾病的认识.方法:回顾性分析2014年1月~2019年5月我院收治的25例抗GBM抗体阳性患者的临床资料.结果:抗GBM抗体阳性患者主要临床表现有肺部病变(咯血等)、消化道症状(腹胀、纳差等)、肾脏病变(水肿、蛋白尿等)、贫血、体重减轻、发热等;单纯抗GBM抗体阳性13例(52.00%),抗GBM抗体伴抗中性粒细胞胞浆抗体(ANCA)阳性7例(28.00%),抗GBM抗体伴抗核抗体(ANA)阳性5例(20.00%);18例患者进行肾活检,新月体型肾炎14例(77.78%);完全缓解3例(12.00%),好转8例(32.00%),无效14例(56.00%),其中11例依赖透析,3例死亡.结论:抗GBM抗体阳性患者具有不同程度的临床症状,病理表现多为新月体型肾炎,预后往往较差.Objective:To investigate the clinical features of anti-glomerular basement membrane(GBM)antibody-related diseases and to deepen the understanding of this group of diseases.Methods:The clinical data of 25 anti-GBM antibody-positive patients admitted to our hospital from January to September 2019 were retrospectively analyzed.Results:The main clinical manifestations of anti-GBM antibody-positive patients were pulmonary lesions(hemoptysis,etc.),gastrointestinal symptoms(blood bloating,anorexia,etc.),renal lesions(edema,proteinuria,etc.),anemia,weight loss,fever,etc.There were 13 cases(52.00%)positive for anti-GBM antibody,7 cases(28.00%)with anti-GBM antibody and neutrophil cytoplasmic antibody(ANCA),and 5 cases(20.00%)with anti-GBM antibody and anti-nuclear antibody(ANA).Renal biopsy was perfonned in 18 patients,14 cases(77.78%)with crescentic nephritis;3 cases(12.00%)with complete remission,8 cases(32.00%)improved,and 14 cases(56.00%)were ineffective.Of these,11 cases relied on dialysis and 3 cases died.Conclusion:Anti-GBM antibody-positive patients have different degrees of clin ical symptoms.The pathological findings are mostly crescentic nephritis,and the prognosis is often poor.
