摘  要:目的:分析数字化种植外科导板在上前牙即刻种植中的临床应用,并评价导板精准度,测量种植体稳定性,为临床术式选择提供参考。方法:选择2018年10月至2019年5月于中国人民解放军总医院第一医学中心口腔种植科就诊,预行上前牙即刻种植的患者共24例。随机将患者分为对照组和试验组,每组12例。两组患者均行即刻种植,试验组在数字化外科导板辅助下进行,对照组常规进行。结果:24例均顺利完成手术,两组患者的临床操作时间(t=0.127,P>0.05)差异无统计学意义;深度测量偏差(t=1.554,P<0.05)差异有统计学意义;冠状面根尖部(t=0.260,P>0.05)颈部(t=0.037,P>0.05)角度(t=1.919,P>0.05)差异均无统计学意义;矢状面根尖部(t=4.921,P<0.05)颈部(t=2.106,P<0.05)角度(t=2.763,P<0.05)差异均有统计学意义。两组患者的种植体稳定性在术后即刻(t=0.180,P>0.05)术后4周(t=0.150,P>0.05)术后8周(t=0.036,P>0.05)三个时间点均无统计学意义。结论:上前牙即刻种植术中,数字化外科导板的应用虽不能明显节省临床操作时间,但有助于提高种植体植入精准度,且在种植体稳定性方面也有良好表现。Objective:Analyze the clinical application of digital implant surgical guide plate in the immediate implantation of the upper anterior teeth,evaluate the accuracy of the guide plate,and measure the stability of the implant,so as to provide reference for the selection of clinical operation.Methods:24 patients preparing for immediate anterior teeth implantation were selected from the General Hospital of the people's Liberation Army in October 2018 to May 2019.Patients were randomly divided into control group and experimental group,12 cases in each group.Two groups of patients were immediately implanted,the experimental group was assisted by digital surgical guide plate,the control group was conventional.Results:All patients operated successfully.There was no difference in the clinical operation time(t=0.127,P>0.05)between the two groups;The depth measurement deviation was significantly different(t=1.554,P<0.05);No different was seen in the neck angle(t=0.037,P>0.05)between the coronal root tip(t=0.260,P>0.05);there was no difference in the sagittal root tip(t=4.921,P<0.05)the angle of neck(t=2.106,P<0.05)was significantly different(t=2.763,P<0.05).There was no difference in implant stability between the two groups immediately(t=0.180,P>0.05),4 weeks(t=0.150,P>0.05)and 8 weeks(t=0.036,P>0.05).Conclusion:Although the application of digital surgical guide plate shows no obvious effect on saving the clinical operation time,it is beneficial to improve the implant accuracy,and show good performance in the implant stability.
