摘 要:目的:探讨在上前牙即刻种植时联合使用钛网和胶原膜及浓缩生长因子(concentrate growth factors,CGF)在修复唇侧骨板重度缺损的骨再生效果。方法:选择30例上前牙即刻种植时存在唇侧骨板重度缺损的患者,应用钛网维持植骨空间,联合使用胶原膜、骨粉和CGF引导骨再生(guided bone regeneration,GBR)。六个月后行二期手术时,观察骨缺损的修复情况,CBCT测量二期手术时及修复后6个月、1年、2年唇侧的骨厚度及边缘骨高度。所获数据进行单因素方差分析和SNK-q检验分析。结果:二期手术时见唇侧骨板重建丰满,种植体顶端少量新骨覆盖,二期手术时、修复后6个月、1年、2年唇侧的骨厚度分别为:3.03±0.45、2.99±0.35、2.97±0.29、2.97±0.35mm,边缘骨高度降低量分别为:高于顶端、0.71±0.21、0.89±0.15、0.89±0.19mm。统计分析显示1年内骨厚度、骨高度变化有统计学意义,而1年后趋于稳定。结论:在上前牙即刻种植重度骨缺损时联合使用钛网、胶原膜和CGF引导骨再生的成骨效果显著、稳定,是一种有效的骨增量方法。Objective:To explore the effect of combining titanium mesh,collagen membrane and concentrated growth factors(CGF)in repairing severe defects of labial bone plate during immediate implantation of maxillary anterior teeth.Methods:Thirty patients with severe maxilla labial bone defect preparing for immediate implantation were selected.The titanium mesh was used to maintain the space of bone graft,and collagen membrane,bone powder and CGF were used to guide bone regeneration.Six months later,when the second stage surgery was performed,the restoration of bone defect was observed.CBCT was used to measure the height of margin bone around the implants and the thickness of bone at the lingual side of the implants at the time of the second stage surgery,6 months,1 year and 2 years after restoration.The data were analyzed by one-way ANOVA and SNK-q test.Results:In the second stage of surgery,the labial bone plate was completely reconstructed,and the top of the implant was covered with a small amount of new bone.The thickness of the labial bone was 3.03±0.45,2.99±0.35,2.97±0.29,2.97±0.35 mm at the time of the second stage operation,6 months,1 year and 2 years after restoration,respectively.The height of the marginal bone around implants was higher than the top,0.71±0.21,0.89±0.15,0.89±0.19 mm at the time of the second stage operation,6 months,1 year and 2 years after restoration,respectively.Statistical analysis showed that the changes of bone thickness and height were statistically significant within one year,but tended to be stable after one year.Conclusion:The results indicate that it is an effective bone augmentation method to use titanium mesh,collagen membrane and CGF to guide bone regeneration in the immediate implantation of maxillary anterior teeth with severe bone defects.
