健脾止泻汤联合美沙拉嗪治疗溃疡性结肠炎54例临床研究

摘　　要：目的:观察健脾止泻汤联合美沙拉嗪治疗溃疡性结肠炎的临床效果。方法:选取108例脾虚湿蕴型溃疡性结肠炎患者作为研究对象,根据随机数字表法分为结合组和对照组各54例。对照组单纯以美沙拉嗪治疗,结合组以健脾止泻汤联合美沙拉嗪治疗。治疗前后分别检测2组患者的炎症因子[白细胞介素-1(IL-1)、肿瘤坏死因子-α(TNF-α)、C-反应蛋白(CRP)]、免疫功能指标[CD3^+、CD4^+、CD4^+/CD8^+],比较2组的临床疗效。结果:治疗后,临床疗效总有效率结合组94.44%,对照组79.63%,2组比较,差异有统计学意义(P<0.05)。2组血清IL-1、TNF-α、CRP水平均较治疗前下降(P<0.05),观察组3项指标水平均低于对照组(P<0.05)。2组CD3^+、CD4^+、CD4^+/CD8^+水平均较治疗前升高(P<0.01),结合组3项指标值均高于对照组(P<0.01,P<0.05)。结论:应用健脾止泻汤联合美沙拉嗪治疗脾虚湿蕴型溃疡性结肠炎患者,可有效减轻炎症反应,增强机体的免疫功能,疗效优于单纯服用美沙拉嗪治疗。Objective:To observe the clinical effect of Jianpi Zhixie tang combined with mesalazine for ulcerative colitis.Methods:A total of 108 cases of patients with ulcerative colitis of spleen deficiency and dampness accumulation type were selected as the study objects and divided into the combination group and the control group randomly,54 cases in each group.The control group was simply treated with mesalazine,and the combination group was additionally given Jianpi Zhixie tang based on the treatment of the control group.Before and after treatment,inflammatory factors including interleukin-1(IL-1),tumor necrosis factor-α(TNF-α)and C-reactive protein(CRP)and immune function indexes including CD3^+,CD4^+and CD4^+/CD8^+in the two groups were detected.The clinical effect in the two groups was compared.Results:After treatment,the total effective rate was 94.44%in the combination group and 79.63%in the control group,the difference being significant(P<0.05).After treatment,the levels of IL-1,TNF-αand CRP in serum in the two groups were all decreased when compared with those before treatment(P<0.05),and the above levels in the observation group were all lower than those in the control group(P<0.05).After treatment,the levels of CD3^+,CD4^+and CD4^+/CD8^+in the two groups were all increased when compared with those before treatment(P<0.01),and the above levels in the combination group were higher than those in the control group(P<0.01,P<0.05).Conclusion:The application of Jianpi Zhixie tang combined with mesalazine for patients with ulcerative colitis of spleen deficiency and dampness accumulation type can effectively relieve the inflammatory responses and enhance the body immune function,whose curative effect is better than that of simple mesalazine.
