不同骨面型双颌前突患者正畸拔牙治疗后唇部软组织变化的临床研究

摘　　要：目的:研究不同垂直骨面型的双颌前突成年女性患者,拔牙矫治后唇位置在矢状向和垂直向的改变。方法:纳入59名已完成拔牙矫治的双颌前突成年女性患者,收集其治疗前后的头颅侧位片影像资料,进行头影测量和统计学分析,评价在正畸治疗中不同垂直骨面型患者的唇部软组织的变化。结果:本研究中所有患者治疗后前牙均得到内收直立,前牙内收的同时没有发生伸长。所有患者上下唇突度明显减小,鼻唇角增加,上下唇均位于E线后方,面部软组织侧貌均得到较大改善。不同垂直骨面型双颌前突患者治疗后面下1/3软硬组织的变化、上下唇矢状向和垂直向变化比例、相关因素不同。结论:拔牙矫治不同垂直骨面型双颌前突患者可协调改善面下1/3骨组织及前牙的形貌,从而改善唇部软组织像貌。Objective:To analyze the sagittal and vertical changes of lip position in adult female patients with different vertical facial skeletal types and bimaxillary protrusion after orthodontic extraction treatment.Methods:59 adult female patients underwent orthodontic extraction treatment were selected in accordance with the inclusion criteria.Pre-and post-treatment lateral cephalograms were used to evaluate the changes of soft tissue of lips in different groups following orthodontic treatment.Results:After treatment,expected anterior teeth retraction without enlongation was achieved in all cases,protrusion of uper and lower lips decreased significantly,nasolabial angle increased,both lips were positioned behind E line,the facial profile improved significantely,the ratio of sagittal and vertical changes of upper and lower lip and the related factors was different in bimaxillary protrusion patients with different vertical skeletal type.Conclusion:Orthodontic extraction treatment may improve the bone and teeth mophology and improve the profile of lips.
