自体牙冠在前牙即刻种植修复中的应用

摘　　要：目的:评估即刻种植后PMMA与自体牙冠2种即刻修复方式的美学效果。方法:12名患者接受共15个牙位的种植治疗,使用Straumann BLT(3枚)和NobelActive(11枚)种植体,其中7个牙位用自体牙冠即刻修复,8个牙位用PMMA牙冠即刻修复。记录并对比2组即刻修复患者在修复体戴入即刻、10 d、5个月的红色-白色美学评分以及患者满意度。结果:2组的红色美学值在3个时间点差异均无统计学意义(F=2.384,P>0.05),不同时间之间比较差异有统计学意义(F=166.935,P<0.05);2组的白色美学值在3个时间点比较差异有统计学意义(F=4.999,P<0.05),不同时间之间比较差异也有统计学意义(F=113.851,P<0.05);自体牙冠组满意度高于PMMA组(F=5.995,P<0.05)。结论:自体牙冠保留了天然牙的解剖形态及自然纹路,利用天然牙根轮廓形态可较好地恢复种植穿龈轮廓及过渡义齿的美学效果。Objective:To compare the esthetic results between PMMA crown and autogenous teeth crown in immediate restoration of dental implantation.Methods:For 12 patients 3 Straumann BLT and 11 Nobel Active implants were respectively used for 15 teeth dental implotation treatment,including 7 with autogenous teeth crown and 8 with PMMA crown.PES-WES scores and satisfaction degree in both groups were recorded and compared on the operation day,10 days and 5 months after operation.Results:There was no significant difference of PES scores between PMMA and autogenous teeth crown groups(F=2.384,P>0.05),while there was significant difference at different visit times(F=166.935,P<0.05);there was significant difference of WES scores between the 2 groups(F=4.999,P<0.05),and at different visit times(F=113.851,P<0.05).Satisfaction degree of autogenous teeth crown group was higher than that of PMMA group(F=5.995,P<0.05).Conclusion:Immediately restoration with the residual autogenous teeth crown provisional prosthesis is a biological sound approach to maintain the soft tissue architecture after teeth extraction,a better way to mimic the contralateral tooth contour and color and a novel method for the satisfaction of patients.
