角膜地形图引导下白内障手术切口构建的选择对术后角膜散光的影响

摘　　要：目的分析角膜地形图引导下白内障手术中角膜切口的选择对患者术后角膜散光影响及术后散光的改变。方法选取2017年1月-12月于我院就诊的白内障患者56例(61眼),依据手术切口不同随机分成常规手术切口位于角膜10点钟方向(对照组);手术切口位于角膜最大屈光轴方向(试验组)两组,其中对照组28例(31眼),试验组28例(30眼)。比较两组研究对象术前、术后1w、1mon、3mon视力和角膜散光度的变化。结果与术前相比,对照组术后1w、1mon、3mon散光度差异无统计学意义(P>0.05),试验组散光度数较术前显著幅度减小(P<0.05),且术后1w、1mon、3mon均低于对照组;两组裸眼视力较术前升高,且试验组术后1w、1mon、3mon裸眼视力均高于对照组(P<0.05)。结论角膜地形图引导下位于角膜最大屈光轴方向行白内障超声乳化手术切口可较好的改善患者裸眼视力、角膜散光度及散光轴向,值得在临床推广应用。Objective To observe the effect of corneal incision guided by corneal topography on corneal astigmatism.Methods A total of 56 patients(61 eyes)with cataract in our hospital from January to December 2017 were randomly divided into two groups:10 point in the conventional cornea(control group)and the incision in the direction of the maximum refractive axis of the cornea(test group),including 28 cases(31 eyes)in the control group and 28 cases(30 eyes)in the test group.Corneal astigmatism and visual acuity were measured before surgery,1 week,1 month,and 3 months after surgery.Results There was no significant difference in astigmatism between the control group at 1 week,1 month,and 3 months after surgery(P>0.05).The astigmatism in the experimental group was significantly reduced compared with that before surgery(P<0.05).And 1 week,1 month,and 3 months after surgery were lower than the control group.The uncorrected visual acuity of the two groups was higher than that of the control group at 1 week,1 month,and 3 months after operation,and the visual acuity of the naked eye was higher in the experimental group than in the control group at 1 week,1 month,and 3 months after operation(P<0.05).Conclusion The cataract surgery guided by corneal topography can improve the visual acuity,astigmatism and astigmatism of the naked eye,especially in the direction of the maximum refractive axis of the cornea.It is worthy of clinical application.
