曲安奈德局部注射联合知柏地黄汤治疗慢性泪腺炎的临床探讨

摘　　要：目的探讨长效糖皮质激素曲安奈德局部注射联合中药知柏地黄汤治疗慢性泪腺炎的效果。方法选取2017年7月-2019年7月昆明医科大学第四附属医院眼科门诊病例50例慢性泪腺炎患者,所有入选患者均为不愿或全身情况不允许大剂量激素治疗,按照随机法将所有患者分成联合组和对照组,每组各25例。其中联合组患者采取曲安奈德局部注射联合知柏地黄汤的方法进行治疗,对照组患者予以曲安奈德局部注射,对比两组患者治疗效果、复发率,以及激素局部治疗次数。结果联合组患者治疗效果优于对照组(P<0.05),且联合组患者复发率低于对照组患者(P<0.05)。在治疗过程中我们还发现,联合组的激素局部治疗次数得到了明显的减少。结论曲安奈德局部注射联合中药知柏地黄汤治疗泪腺炎对于不愿接受或全身情况不允许大剂量激素治疗的患者是一种可行而有效的方法。Objective To explore the effect of local injection of triamcinolone acetonide combined with Zhibai Dihuang Decoction in the treatment of chronic lacrimal gland inflammation.Methods 50 patients with chronic lacrimal gland inflammation who were admitted to the ophthalmic outpatient department of the Fourth Affiliated Hospital of Kunming Medical University from July 2017 to July 2019 were selected as the research object.All the selected patients were unwilling or not allowed to be treated with high-dose hormone in general condition.According to the random method,all patients were divided into the combined group and the control group,25 cases in each group.The patients in the combination group were treated by local injection of triamcinolone acetonide combined with zhibai dihuang decoction,while the patients in the control group were given local injection of triamcinolone acetonide,and the treatment effect,recurrence rate and local hormone treatment times were compared between the two groups.Results The treatment effect and recurrence rate of the combined group were better than those of the control group(P<0.05).In the course of treatment,we found that the number of hormone local treatment in the combined group was significantly reduced.Conclusion Triamcinolone acetonide local injection combined with Zhibai Dihuang decoction is a feasible and effective method for patients who do not want to accept or do not allow high-dose hormone treatment.
