益气逐瘀汤对分水岭区脑梗死气虚血瘀证患者PAF、MMP-9的影响

摘　　要：目的探讨益气逐瘀汤对分水岭区脑梗死气虚血瘀证患者血小板活化因子(PAF)、基质金属蛋白酶(MMP-9)的影响。方法回顾性分析北京市房山区中医医院2017年2月至2019年2月接受诊疗的104例分水岭区脑梗死且存在气虚血瘀证患者,依治疗方法不同分为常规组和中医组,各52例。常规组行常规治疗,中医组在此基础上加施益气逐瘀汤治疗。治疗周期为2周。比较2组患者临床治疗效果、治疗前后PAF、MMP-9、C反应蛋白(CRP)水平及美国国立卫生研究院卒中量表(NIHSS)评分、Barthel指数。结果治疗后中医组患者临床治疗总有效率96.15%明显高于常规组82.69%(P<0.05)。与治疗前比,治疗后2组患者血清PAF、MMP-9、CRP水平均明显下降,且中医组明显低于常规组(P<0.05)。与治疗前比,治疗后2组NIHSS评分均明显降低,且中医组明显低于常规组(P<0.05);治疗后2组Barthel指数均明显上升,且中医组明显高于常规组(P<0.05)。结论益气逐瘀汤可明显提高分水岭区脑梗死气虚血瘀证患者的临床治疗疗效,控制血凝,促进患者脑部血液流速,改善患者神经功能缺损情况。Objective To explore the effect of Yiqi Zhuyu Decoction on PAF and MMP-9 in patients with cerebral infarction of qi deficiency and blood stasis syndrome in watershed.Methods A total of 104 patients with cerebral infarction in watershed area and Qi deficiency and blood stasis syndrome were retrospectively selected from February 2017 to February 2019 in Beijing Fangshan District Hospital of traditional Chinese medicine.According to the different treatment methods,they were divided into conventional group and traditional Chinese medicine group,52 cases each.The routine group was treated with routine therapy,and the traditional Chinese medicine group was treated with Yiqi Zhuyu Decoction.The treatment period was 2 weeks.The clinical effect,PAF,MMP-9,CRP,NIHSS and Barthel index were compared before and after treatment.Results After treatment,the total effective rate of TCM group was 96.15%,which was significantly higher than that of routine group 82.69%(P<0.05).After treatment,the levels of PAF,MMP-9 and CRP in the two groups were significantly lower than those before treatment,and those in the traditional Chinese medicine group were significantly lower than those in the conventional group(P<0.05).Compared with before treatment,NIHSS scores of the two groups were significantly lower after treatment,and the traditional Chinese medicine group was significantly lower than the conventional group(P<0.05);Barthel index of the two groups was significantly higher after treatment,and the traditional Chinese medicine group was significantly higher than the conventional group(P<0.05).Conclusion Yiqi Zhuyu Decoction can obviously improve the clinical therapeutic effect of cerebral infarction in watershed area,control blood coagulation,promote blood flow velocity in brain,and improve the condition of nerve function defect.
