穴位贴敷按摩联合5-HT3受体拮抗剂对恶性肿瘤患者化疗后呕吐及TNF-α、IL-6水平的影响

摘　　要：目的 探究穴位贴敷按摩联合5-羟色胺(5-HT3)受体拮抗剂对恶性肿瘤患者化疗后呕吐及血清肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)水平的影响.方法 选取无锡市中医医院2016年1月至2019年7月收治的60例恶性肿瘤化疗患者为研究对象,按照随机数字表法分为对照组和观察组,各30例.对照组予以5-HT3受体拮抗剂治疗,观察组则联合穴位贴敷治疗,治疗周期均为6周.比较2组治疗前后血清TNF-α、IL-6水平与呕吐生活功能指数(FLIE)、国际指南推荐的罗德恶心及呕吐指数评估量表[Rhods Index of CINV(R-INVR)]评分以及治疗后呕吐有效控制率.结果 与治疗前相比,2组治疗后血清TNF-α、IL-6水平及R-INVR评分降低,且观察组低于对照组(P<0.05);2组治疗后FLIE升高,且观察组高于对照组(P<0.05);治疗后观察组有效控制率100%高于对照组62.50%(P<0.05).结论 采用穴位贴敷按摩联合5-HT3受体拮抗剂治疗可通过降低TNF-α、IL-6水平有效缓解恶性肿瘤患者的化疗性呕吐,值得推广.Objective To explore the effect of acupoint application and massage combined with 5-HT3 receptor antagonist on vomiting and serum TNF-α,IL-6 levels in patients with malignant tumor after chemotherapy.Methods Totally 60 patients with malignant tumor undergoing chemotherapy in wuxi hospital of traditional Chinese medicine from January 2016 to July 2019 were selected as the research objects.According to the random number table method,they were divided into control group and observation group,with 30 patients in each group.The control group was treated with 5-ht3 receptor antagonist,and the observation group was treated with acupoint application.The treatment cycle was 6 weeks.Serum TNF-and il-6 levels before and after treatment were compared with FLIE,r-invr scores and the effective control rate of vomiting after treatment.Results Compared with before treatment,serum TNF-,il-6 levels and r-invr scores of the 2 groups decreased after treatment,and the observation group was lower than the control group(P<0.05).FLIE increased in the 2 groups after treatment,and the observation group was higher than the control group(P<0.05).After treatment,the effective control rate of the observation group was 100%higher than that of the control group(P<0.05).Conclusion Acupoint application and massage combined with 5-HT3 receptor antagonists can effectively alleviate chemotherapy-induced vomiting in patients with malignant tumors by reducing TNF-αand IL-6 levels,which is worth popularizing.
