老年肿瘤患者多重耐药菌院内感染危险因素分析

摘　　要：目的探讨高龄恶性肿瘤患者医院感染多重耐药菌的现状及相关危险因素。方法回顾性分析我院老年科2014—2018年收治的恶性肿瘤患者资料,对比多重耐药菌感染组及无多重耐药菌感染组间的差异,并对可能的危险因素进行统计分析。结果共284例入组病例,年龄58~102岁,平均(87±8)岁,发生院内多重耐药菌感染者111例,感染率39.1%,共检出耐药菌株145株;年龄、衰弱情况、住院时间长、合并基础疾病多、有侵入性操作是老年恶性肿瘤患者合并院内多重耐药菌感染的独立危险因素。结论多重耐药菌感染是高龄肿瘤患者易出现的严重并发症,高龄、基础疾病多等不可控因素可增加肿瘤患者的多重耐药菌感染率。同时,针对高龄肿瘤患者衰弱带来的活动减少、长期卧床、跌倒误吸等负性事件等可控因素进行防控可能对于降低患者的耐药菌感染率,减少其带来的不良结局有更重要的意义。Objective To further understand the current statusand risk factors of prevalence of nosocomial infections caused by multidrug-resistant organisms(MDROS)in elderly patients with malignant tumor.Methods A retrospective study was performed to analyze the clinical data of patients with malignant tumor admitted to the Department of Geriatrics in our hospital from 2014--2018,the differences between patients with and without MDROS infections were compared and possible risk factors were analyzed.Results A total of 284 patients aged between 58 and 102 were enrolled in this study,with an average age of 87±8.Among them,111 cases were infected by MDROS,with the infection rate of 39.1%.The major risk factors for the MDROS infections include age,frailty,long hospital stay,multiple concomitant chronic diseases,and invasive procedures.Conclusion MDROS infection is a serious complication of elderly patients with malignant tumors.Uncontrollable factors such as age and multiple concomitant chronic diseases can increase the infection rate of MDROS.At the same time,the prevention and control of adverse events caused by frailty in elderly patients may have more important significance in reducing the adverse outcomes and infection rate of MDROS.
