摘 要:目的探讨经侧裂-岛叶入路清除优势侧基底核区脑出血(ICH)对患者语言功能的保护作用。方法将安徽医科大学附属巢湖医院神经外科2014年1月-2018年12月入院的优势侧基底核区脑出血61例患者随机分为经侧裂-岛叶入路30例(观察组),经颞中、上回入路31例(对照组),比较两组患者语言功能及预后。术后3个月采用波士顿诊断性失语检查量表(BDAE)对患者语言功能进行评估,并采用格拉哥预后评分(GOS)对患者预后进行评分。结果术后3月观察组失语程度分级高于对照组(P<0.05);观察组流畅、复述及命名的恢复均好于对照组(P<0.05),理解在两组差异无统计学意义(P>0.05);预后良好率两组无明显差别(P>0.05)。结论经侧裂-岛叶入路清除优势侧基底核区脑出血,可减轻患者失语症状,提高患者生存质量。Objective To investigate the protective effect of transtemporal-insular approach on language function in patients with basal ganglia Intracerebral hemorrhage in dominant side.Methods 61 patients with basal ganglia intracerebral hemorrhage in dominant side admitted to our department from January 2014 to December 2018 were randomly divided into 30 patients through the lateral temporal-insular approach(observation group)and 31 patients through the middle and upper temporal gyrus approach(control group).The language function and prognosis of the two groups were compared.The language function was assessed by the Boston diagnostic aphasia examination(BDAE)3 months after operation,and the prognosis was assessed by the Glasgow Outcome Scale(GOS).Results Three months after operation,the aphasia grade of the observation group was higher than that of the control group(P<0.05);the recovery of fluency,repetition and naming of the observation group was better than that of the control group(P<0.05),and there was no significant difference in understanding between the two groups(P>0.05);there was no significant difference in the good prognosis rate between the two groups(P>0.05).Conclusions Removal of basal ganglia Intracerebral hemorrhage in dominant side via lateral temporal-insular approach can alleviate aphasia symptoms and improve the quality of life of patients.
