摘　　要：目的探讨内镜下套扎(EVL)联合内镜下硬化剂注射(EVS)治疗对肝硬化食管胃底静脉曲张患者止血成功率及复发率的影响。方法选择2018年1月至2019年10月我院收治的60例肝硬化食管胃底静脉曲张患者,随机分为观察组30例和对照组30例。对照组采用EVL治疗,观察组在此基础上联合EVS进行治疗。对比2组临床疗效以及并发症发生情况。结果治疗后,观察组止血成功率、静脉曲张消失率略高于对照组,但差异无统计学意义(P>0.05);相比对照组,观察组再出血率、静脉曲张复发率均更低,差异有统计学意义(P<0.05);较对照组,观察组低热、溃疡出血、吞咽梗阻以及胸骨后疼痛发生率均略低,但差异无统计学意义(P>0.05)。结论EVL与EVS连用治疗肝硬化食管胃底静脉曲张患者可以有效提高止血成功率,降低静脉曲张复发率,且不会增加并发症发生率,临床效果确切,值得临床推广应用。Objective To investigate the effect of endoscopic variceal ligation(EVL)combined with endoscopic variceal sclerotherapy(EVS)on success rate of hemostasis and recurrence rate in patients with liver cirrhosis esophageal and gastric varices.Methods Sixty patients with liver cirrhosis esophageal and gastric varices who admitted in the hospital from January 2018 to October 2019 were randomly divided into two groups,with 30 cases in each group.Control group was treated with EVL,and observation group was treated with EVS based on control group.The clinical efficacy and the incidence of complications were compared between two groups.Results After treatment,the success rate of hemostasis and disappearance time of varicosity in observation group were slightly higher than those in control group,but the difference was statistically significant(P>0.05);The rebleeding rate and recurrence rate of varicosity in observation group were lower than those in control group(P<0.05);The incidence of lower fever,ulcer bleeding,swallowing obstru-ction and retrosternal pain in observation group was slightly lower than that in control group,but the difference was not statistically significant(P>0.05).Conclusion EVL combined with EVS in treatment of patients with liver cirrhosis es-ophageal and gastric varices can increase the success rate of hemostasis,decrease the recurrence rate of varicosity,and not increase the incidence of complications,which is effective and worthy of clinical promotion and application.
