扶正化痰解毒方干预恶性淋巴瘤免疫逃逸作用临床观察

摘　　要：目的:观察扶正化痰解毒方干预恶性淋巴瘤肿瘤负荷、免疫逃逸作用机制及安全性。方法:共收入初治或复发恶性淋巴瘤患者123例,随机分为观察组62例和对照组61例。所有入组患者采用R-CHOP/CHOP行4疗程,21天或28天为1周期,观察组同时予扶正化痰解毒方。观察化疗4疗程前后中期评估、肿瘤负荷、免疫逃逸指标、中医证候积分、安全性指标。结果:两组经治疗后,LDH、SF、血β2微球蛋白、CD3、CD4指标4疗程前后对比差异有统计学意义(P﹤0.05)。两组TGF-β1、IL-10治疗后水平没有统计学差异,但是差值有差异。两组中期疗效差异无统计学意义,总有效率观察组高于对照组。两组中医证候积分治疗后差异有统计学意义。血常规的波动水平治疗前后差异有统计学意义。余肝肾功能、心电图、尿粪常规水平变化差异均无统计学意义。结论:扶正化痰解毒方能降低恶性淋巴瘤的肿瘤负荷,其机制可能与调控肿瘤的免疫逃逸有关。Objective:To observe the mechanism and safety of Fuzheng Huatan Jiedu recipe on malignant lymphoma-caued tumor load and immune escape.Methods:A total of 123 patients with malignant lymphoma who were newly treated or relapsed were randomly divided into observation group(62 cases)and control group(61 cases).All patients of two groups were treated by R-CHOP/CHOP for 4 courses of treatment,21 or 28 days as a cycle;at the same time,the observation group was treated plus with Fuzheng Huatan Jiedu recipe.The mid-term assessment,tumor load,immune escape index,TCM syndrome score and safety indexes were observed before and after 4 courses of chemotherapy.Results:After two groups were treated,the indexes of LDH,SF,bloodβ2 microglobulin,CD3 and CD4 before and after 4 courses of treatment were compared,the difference was statistically significant(P﹤0.05).After treatment,in the levels of TGF-β1 and IL-10,there was not statistical difference between two groups,but the difference values were significant.The difference of mid-term efficacy between two groups was without statistical significance.The total effective rate of observation group was higher than that of control group.After treatment,the difference of TCM syndrome score between two groups was statistically significant.Before and after treatmen,the difference of fluctuating level of blood routine was statistically significant.In liver-kidney function,electrocardiogram and urine-feces routine level,the changing difference was without statistical significance.Conclusion:Fuzheng Huatan Jiedu recipe can reduce the tumor load of malignant lymphoma.Its mechanism may be related to regulating the immune escape of tumor.
