两种手术方式治疗青光眼并白内障的疗效对比

摘　　要：目的研究单纯小梁切除手术和小梁切除术联合人工晶状体植入术+超声乳化白内障吸除术对青光眼合并白内障进行治疗的效果。方法本文选择我院在2017年3月至2018年10月收治的青光眼并白内障患者78例作为研究对象,按随机方法将所有患者分为A组和B组,平均每组患者分别为39例。治疗过程当中,A组应用小梁切除术进行治疗,B组患者则采用小梁切除术联合人工晶状体植入术+白内障吸除术进行治疗,对两组治疗以后的效果进行评价和比较。结果对两组治疗的成功率进行比较,A组为84.62%(33/39),B组为97.44%(38/39),P<0.05,差异具有统计学意义;比较两组并发症发生率,A组为20.51%(8/39),B组患者为20.51%(8/39),P>0.05,不存在统计学意义;对两组治疗前后的视力和眼压状况进行评价比较,两组治疗之前无明显差异性,P>0.05,治疗以后,B组明显优于A组,P<0.05,差异具有统计学意义。结论对于青光眼并白内障患者在治疗时选择小梁切除术联合人工晶状体植入术+超声乳化白内障吸除术进行治疗可以取得良好治疗效果,能提高手术成功率,并且改善患者的视力状况,值得推广。Objective To investigate the effect of trabeculectomy and trabeculectomy combined with intraocular lens implantation and phacoemulsifi cation cataract aspiration on the treatment of glaucoma complicated with cataract.Methods 78 cases of glaucoma and cataract patients admitted to our hospital from March 2017 to October 2018 were selected as the research objects.All patients were randomly divided into group A and group B,with an average of 39 cases in each group.In the process of treatment,trabeculectomy was applied in group A,and cataract extraction was performed in group B with trabeculectomy combined with iol implantation.The results of the two groups were evaluated and compared.Results The success rate of treatment was compared between the two groups,84.62%(33/39)in group A and 97.44%(38/39)in group B,P<0.05,and the difference was statistically signifi cant.The incidence of complications was compared between the two groups,with 20.51%(8/39)in group A and 20.51%(8/39)in group B,P>0.05,indicating no statistical signifi cance.The visual acuity and intraocular pressure status of the two groups before and after treatment were evaluated and compared.There was no signifi cant difference between the two groups before treatment(P>0.05).After treatment,patients in group B were signifi cantly better than those in group A(P<0.05),and the difference was statistically signifi cant.Conclusion Trabeculectomy combined with intraocular lens implantation and phacoemulsifi cation cataract implantation for glaucoma and cataract patients can achieve good therapeutic effects,improve the success rate of surgery,and improve the visual acuity of patients,which is worthy of promotion.
